Title: Living in Emergency (LiER) ~Dec 24~

A product of my procrastination during my break time and down time at work. This is more of something to satisfy my urge to write and a form of entertainment for myself when I'm in my bored moments and don't want to study or do my assignments. Just a warning there might be a LOT of medical term usage and things going on and it will get confusing, I made up a little index with some terms that I will be using frequently and will be updating that as I go. For those that are brave enough to continue on with my craziness please refer to the index to see if it will help explain things out a bit.

Most of the cases that do appear in here are things that I have come across at work mixed with a bit of creativity and imagination. I'm not sure how long this little project will live, probably as long as I have things that bug me at work. There is no set plot for this so this will be a "go with the flow" stories, and the updates will be written on the spot.

Living in Emergency: PAGE 1



“Respiratory STAT to emergency! Respiratory STAT to emergency!”

Sound of running can be heard ringing down the halls, patients and visitors look up only to see a blur running past them. If not for the blaring overhead announcements they would definitely say it was their imagination playing tricks on them as the image left before it can be registered in their brain.

“Respiratory is on their way doctor,”

“What is the status on the ambulance and patient?” asked Dr. Takahashi as she arrived in the trauma bay.

“They should be here any minute now. Post cardiac arrest found at home by granddaughter who called ambulance. CPR started on arrival by ambulance crew, they were unable to secure a breathing tube on scene.” Kuramochi Asuka, the head emergency nurse quickly summarized the situation.

“Respiratory here, what’s the deal?” asked Sashihara Rino, the in hospital respiratory therapist (RT), who came running through the doors with her partner in crime Minegishi Minami.

“Post arrest coming, please set up for intubation.” Dr. Takahashi yelled out over her response team that are all ready for action.

“Male or female?” Sasshi asked as she started pulling out different sizes of breathing tubes and equipments that will be vital in the next couple minutes.

“Female,” Mocchi replied as she took another look outside to the ambulance loading area, “they are pulling up so any seconds now,” she announced to the team.

The sounds of wheels rolling into the trauma bay can be heard as the ambulance crew came rushing through the doors. A totally of four crew members can be seen, one at the end pushing the stretcher, while another is at the head of the stretcher guiding its path holding onto a chart that contains the patients history. The third one can be seen almost straddling the patient on top of the stretcher performing compressions on the rolling stretcher while the last was at the side of the patients head manually ventilating the patient with a bagger through a face mask.

Once they stopped, the paramedic on the stretcher jumped down and quickly moved out of the way as they transferred the patient onto the hospitals stretcher.

“Alright on 3, 2, 1,” proficiently the paramedics moved the patient over the bigger stretcher as the nurses wasted no time to start connecting the monitors and pads at the same time one can be seen getting in place to continue CPR.

In perfect harmony, the team weaved around each other working against the clock, hoping to bring life back and restart the heart of this poor little lady. Without missing a beat the paramedic recited the patient’s condition and history for everyone present to hear.

“71 year old female found collapsed in the bathroom by her granddaughter, who called ambulance. On scene, we started CPR immediately after finding no pulse, shocked once and rhythm returned with pulse, we were able to get one IV in before we lost her pulse again”

*Dr. Takahashi’s POV*

“Okay let’s do this, continue CPR, and have Epinephrine and Atropine ready!” I ordered, “Sasshi, you intubate the patient,”

I watch Sasshi moved to the head of the bed with the breathing tubes in hand as Miichan stood by her side with their needed equipments for the procedure. Once everything is laid out, Miichan relieved the paramedic and took over manual ventilation until Sasshi is ready to insert the breathing tube, with one hand holding a mask over the patients face, she squeezing the bagger with her other to deliver another breath into the lungs. As the RT’s busy themselves with securing the airways, our skillful nurses, Takajo Aki and Yokoyama Yui, took turns doing compressions. Each of their compressions aimed to circulate blood to preserve the body, by circulating oxygenated blood to keep the brain and organs alive until they are able to restart the heart.

“Dr. Takahashi, we still don’t have a pulse, patient is in PEA (Pulseless Electrical Activity)!” Sato Amina spoke out loud and clear for the team as she paused in her charting to check the pulse.

“Keep up the CPR, Yui give the patient 1mg of Epi IV.” I instructed, watching them quickly carry out their tasks.

With one swift motion Sasshi had the patient’s head repositioned and is looking down her throat with a lighted blade in one hand while pushing the breathing tube into the lungs with the other. Miichan quickly made sure that they have the breathing tube in the lungs and not in the esophagus, before securing the breathing tube.

“Good air entry heard in the lungs, we are in Dr. Taka, airway is secured.” Sasshi reported.

“Dr. Takahashi, 1mg Epi is in.” after reporting to me Yui and Akicha switched position again with Yui performing CPR, I can see Akicha shaking out her arms a bit.

“Stop, check for pulse again.” I watch Amina pause from her recording again and quickly moved to check for a femoral pulse.

“We have a pulse!” she spoke up loud and clear for everyone to hear.

“Stop compressions, get another IV in and give the patient 500mL of fluid IV immediately, get Dr. Shinoda on the phone and alert ICU we have a patient for them and will need to be started on cooling protocol.” I gave out another set of orders as I move to assess the patient.

“Is family here?” I asked as I check over the patient again, her prognosis is poor and given her downtime there is a chance she won’t make it.

“Her granddaughter is in the waiting room,” Mocchi replied.

I really hope that "granddaughter"  :roll: :roll: is MAEDA ATSUKO..... :inlove: :inlove: :lol: :lol: so that the story will become ATSUMINA Emergency... loving each other...  :heart: :heart:

Interesting story... I feel like watch Grey's Anatomy!

Takamina is not a surgeon right? So... Grey's Anatomy didn't fit! It's just my stupid imagination :doh:

OMG KIRA!!!

Finally some update :P
and this is so exciting!!!!!!! Though some of the medical terms that you use, I don't know it. Thank GOd you posted the meaning of those terms :)

I hope that 'grandaughter' is ACCHAN too!!!! or not...I want her to be TAKAMINA'S NURSE...you what I mean ;)

I cant wait for more!!!!

Hee, a medical fic. It's like reading a Team Medical Dragon or Godhand Teru.

I won't expecting there's some pairing. And I won't request for one. I'll enjoying how Dr. Takahashi and the staffs saving peoples life.

I'm waiting for the next installment.

@immortal_K: another comment!  :onioncheer: this fic reminds me of Grey's Anatomy... but more action in Trauma Bay instead of the Surgery Wing :3
I can imagine Dr. Takamina leading the team calmly... when I watch medical drama it's always heart pounding and panicky  :dizzy: it's good that grandma in the story is okay... and now me, like the rest of the readers are now wondering who the grand daughter is.  :nervous

aoi_sora: LOL Aamin.. hmmm I will have her in the story for sure  :lol: I guess you can say it is like Grey's Anatomy, but then that show has more drama in relationships then the actual care and treatment HAHA

blueangel65: Wow so many Atsumina predictions, I hope you are still up for reading after so long.

So after a very long time I finally have time to write up another page of LiER at work  :lol: I am quite surprise with all the comments I got for this story that I never expect people will find interesting with the millions of hospital/medical stuff that I have added into the writing. As I have mention before, this fic is originally for my self to just spill out things that I have seen and done or heard of at work. Most cases are modified to fit the story but its not fake, and yes every case that will appear I have been there actively participated in the treatment and care with the rest of the team. Personally I believe in the teamwork, I believe that it takes more then just the doctor or the nurses or the RT's etc to provide optimal care and best treatment.  So a 'THANKS' to all those health care professionals out there.

This is definitely one of the longest updates I've ever written.

         ~If everyday is a new page in life, what chapter are you at in your story~



*Takahashi Minami’s POV*


I always dreaded this part of the job the most, I have no problem performing the procedures and caring for the patient, it is speaking with the family that I always have trouble with. I stood outside the family waiting room, hesitating to enter as I can already hear the sobbing coming from inside the room. Just hearing this already have my heart wrenching in pain unconsciously putting myself in their position and feeling the pain they are in. Sometimes this helps me gain a level ground, so I can speak to them in their terms while still keeping a clear mind of the situation.

Pausing for another minute I finally reached to turn the knob to the door, if I stand here any longer I might start getting unwanted stares from the patients and nurses. The sound of opening door caused the sobbing to pause, even before I fully stepped into the room I was bombarded with questions.

“How is she? Will she be alright? Is she going to make it through? This is all my fault!” question after questions were thrown towards me by the granddaughter, all while keeping her head down.

“Calm down and I will slowly explain to you what is going on, have a seat first.” I spoke with the calmest voice I can manage while placing one hand on her shoulder as a form of support. I took out a pack of tissues and pass it to the girl waiting for her to take it and dry her tears.

Finally she lifted her head so I can get a good look at her face, those eyes that stare back at me… I know that face.

“Yuko?”

She stopped crying and started looking at me with this confused look, like a lost puppy or maybe a squirrel… Moments pass by with this awkward silence surrounding us with my hand still on her shoulder while the other one outstretched and still holding onto the pack of tissue.

“Taka-midget?”

That was all it takes for me to confirm she is who I think she is, no one uses that name for me anymore, well at least not together. I was only able to respond her with a nod before I was attacked by a flying squirrel who decided to borrow my shoulders for support and cry on. It didn’t even take long for me to start feeling the wet spot on my shoulder but I wasn’t about to just push Yuko off of me, in contrast I allowed myself to snake my arms around her for a reassuring hug. Tired of standing in the middle of the room with the door still half ajar, I quickly swatted the door closed before moving over to one of the couches. I continue to comfort the crying girl, giving her occasional whispers of encouragement until she is ready to listen to what I have to say. When I felt that Yuko is composed enough for us to talk and discuss her grandma’s situation, I untangled myself and turned over to look at her, making sure to maintain eye contain with the best positive body language I can muster up.

“Yuko, I’m Dr. Takahashi, I was in charge of your grandma’s resuscitation when she was brought into our emergency room.” I start off by introducing myself even thought I suddenly felt a bit silly to introduce myself to someone that I use to talk to on a first name basis. I waited for her to let the words sink in before continuing.

“As you know, your grandma’s heart stopped, but we were able to bring it back. We had to put a breathing tube in to help her breathing because she isn’t able to do that on her own right now. She is also connected to monitors that help us watch her condition while she receives medication given to her through lines that goes straight to her veins.” I paused again to allow her some time to digest all that I have said.

“Will she be okay?” Yuko asked, with hope in her eyes.

“For now she is stable, we will transfer her into intensive care unit for management and care, the doctor and nurses there will take good care of her. We will try our best to do what we can but you have to know this, her heart did stop for a period of time and we can’t predict the amount of damage and impairment it might have caused. BUT we will do our best.” I made sure to emphasize our position in her care to settle Yuko’s mind a bit with her knowing her grandma is in good hands.

“It is all my fault... *sniff* if only I...*sniff* started CPR when I found her lying on the floor in the bathroom…I just blanked out.” Yuko started crying again, “If only I concentrated on my studies instead of always running around causing trouble.”

Once again I placed an arm around her shoulder in a half hug allowing her to wet the other side of my shirt. “Shuuuu, it’s not your fault you were in shock, and you remember to call for help. If you didn’t do that then she won’t even be here anymore.” I spoke gently trying to calm the distress girl.

“But I’m always causing her trouble, getting into fights… she’s all I have left…” tears continue to pour out of her already red and puffy eyes.

I sat there with Yuko in that exact position until I notice her breathing becoming shallower and slower. Using my hand to move her hair to aside I see her eyes now closed as she had cried herself to sleep, energy all spent with traces of tear marks on her face. I debated if I should excuse myself and head back to the emergency room as my duty is to care for those that are sick but seeing how nobody came to check on me, they should be managing the workload. At most the Maeda sisters might just demand a free dinner for doing my share of work, I’m sure I can manage them one dinner, it’s not like I can get away from them anyways.

Dr. Shinoda Mariko, Intensive Care doctor, trained in Internal Medicine and Respiratory Medicine. Studied and practiced in USA, she was one of the top in her class, returning last year to take up a position as one of the ICU doctors. Although beautiful for the eye with looks of a professional model, her personality can occasionally be quite sadist for her staff. Rumors say she goes around trolling all her ICU nurses causing them to resign because they can’t take the constant abuse.

“Wow Mariko, only a year and you already build up quite a reputation, you are so well known here for being the one doctor to avoid at all cost,” two tables down from the gossiping nurses the ICU doctors sat trying to enjoying their coffee. At least one of them was until that peaceful atmosphere was broken by the individual doctor who now has her hands covering her mouth trying to muffle her laugh at her friend’s expense.

“Gorilla, shut up” the icy tone and intense glare intended to silence Sayaka had as much effect as giving a drug user a pinch of morphine. Sayaka just doubled over and tried harder to muffle a second wave of laughing, once settled she lift her head and flashed her friend an awkward smile as she tried to prevent another laughing fit. Mariko attempted to counter it with a deadly glare, but soon it turned into a smirk as the gossiping nurses continue with their live bio broadcast.

Dr. Akimoto Sayaka, director of ICU also trained in Internal Medicine and Respiratory Medicine specialist, also the Head of the department of Medicine and a consultant for the Center for Disease and control. Known as the only daughter of the founder of AKB, she will succeed her father and eventually take over the management. Rumor says Dr. S. Akimoto is strong as a Gorilla and her abs are as hard as a washboard; she is a slave driver and has very high expectations for her staff. Her glares and lectures bring her staffs to their knees breaking even the strongest willed individual.

Sayaka frown after hearing what is being said about her, “You should smile more and don’t glare, we are short on staff, new graduates are scared to apply to the hospital, because we are known to have a scary doctor,” Mariko said with a smirk.

“Well, you should stop trolling them, we lost so many nurses because of your sadist nature,” Sayaka retaliated.

“ICU is fully staffed right now, you don’t see them being scared away” retorted Mariko.

“Correction, ICU is the only unit that is fully staffed, and that is because all the ones that couldn’t stand you…”

“Us” Mariko cut off Sayaka, just to remind her they are in it together.

“the ones that couldn’t stand us left after the first couple months, the one’s WE have left are crazy.” Sayaka finished.

“They aren’t crazy they are just the best of the best.” defended Mariko.

“You are only saying that because Haruna is a part of the ICU staff.” Sayaka got up and headed out of the cafeteria, throwing away her empty coffee cup on the way out. Mariko just sat there starring out the window watching the rain pour out of the sky.

*Beep* *Beep*

Mariko took out her pager to check the number, seeing the local to Emergency she quickly picked up her cell to return the call.

“Hi this is Dr. Shinoda… okay I will be there shortly.”

“Hey Mocchi, where is the little one?” Mariko asked as she strolls into emergency right next to the head nurse while peering through the curtains to get a glimpse of her soon to be patient.

“Dr. Shinoda, Dr. Takahashi went to the waiting room to speak with the family,” Mocchi replied as she continues to fix up the charting to the resuscitation they had performed earlier.

“Alright, I’ll assess the patient first and write you some orders so we can get her moving to ICU as soon as we can. Did we get the chest x-ray for breathing tube placement?”

“Yes, that has been done,” Mocchi replied as she watched Mariko move over to patient’s bedside to start her patient assessment.

“Mocchi, get the papers together we will move this one to ICU first. I want to start cooling her to improve the chances of recovery while minimizing chance of anoxic brain injury. I’ll finish the orders over there.” Mariko told the nurse in charge, “I’ll go ahead and inform ICU so you don’t have to worry about calling them.”

“Roger that, Mariko-sama,” Mocchi said with a smile towards the departing doctor.

“That’s Dr. Shinoda to you,” Mariko joked before walking out the doors of the unit.

“Yui can you page Sasshiko back, we need her to transport the patient to ICU right now. Akicha can you get the portable monitor ready and Amina please draw up some morphine and midazolam, just incase we need something on the way.” Mocchi quickly delegated her staff to get the patient ready for moving.

*Sashihara Rino’s POV*


~After disappearing from the Emergency Room~

“Hi, my name is Sashihara, I am a Respiratory therapist, I came to check your breathing, your nurse told me you were having some trouble earlier.” I introduced myself while analyzing the patient in front of me, his chart said, he is 45years old, came in with pneumonia, some drug and alcohol abuse, depression and history of psych consult.

“How are you today?” I asked.

“Me? I don’t know, I saw Ronald McDonald chasing me with purple and yellow teletubbies behind him earlier today.”

‘Wow what has this guy been smoking’ I thought as I stared at the guy in front of me.

“You are fine now, they are gone. Do you know where you are and what day it is?” I asked to check and see if he is orientated at all.

“Today, is pay day, I usually get paid every other week on Monday, I have been waiting for today since last week.” he replied.

Yup today is Monday alright, at least he knows the day...

“Hmm you look weird” he suddenly said while looking at me, then he turns and look around the room, “nice place you have here, how much did you pay for rent? I like the colors of the wall.”

I don’t know how to respond, first he calls me weird then he comments on the nice hospital walls..... “Mister you are at Akibahara Hospital, this is a hospital room, it is not for rent but you do have to pay for staying here. I’m going to have a listen to your lungs.”

I moved to place my stethoscope on his back over where the lungs are located and heard some low rattling sounds of crackles and some wheezes. Before I was able to remove my stethoscope from my ears heard some muffled sound “Sorry, you were saying?”

“You look my age, how about we go out and have a nice dinner together?” he asked.

Okay this is so funny and freaky at the same time, first he tells me he was chased by teletubbies, then calls me weird and now he just asked me out for dinner.

“I’m sorry, you can’t leave the hospital yet, I am going to give you some Ventolin and Atrovent, its breathing medications that help open up your airways and make your breathing easier.” I quickly grab one of the medication masks left on the wall and connected it to the wall oxygen flow meter and started it. Once I see the mist of medication coming out I placed it over the patients face.

“WOW, this is pretty cool, feels nice, maybe I should get one of these for myself, it even comes with a tubing to hold it with.” he said while playing with the mask and tubing.

“You can have that mask, for free, just take it with you when you go home.” I said before I slowly eased my way out of the room.

“Miss, what about dinner tonight? I got paid today, my treat.” he asked again.

“I’m busy.” I said before making a run out of the room to letting the nurse know I started him on some bronchodilators.

*Beep* *Beep*

I checked the number on my pager and called back to emergency. “Hello, Respiratory Sasshi…..okay I’ll be right there.”

I quickly hung up the phone and made my way back to emergency for transporting the patient to ICU.

Finally! you posted an updated for this fic!  :on GJ: I love how you were able to write Takamina's POV... I can really imagine what she was doing! I was  :shock: in a good way when I found out that Yuko is the granddaughter.  :twothumbs Mariko and Sayaka's scene is great too! Honestly, I would love to go to a hospital that has the AKB members as their doctor's and nurses  :luvluv1:

caghaunt: Yes, Team Medical Dragon, Godhand Teru, Code Blue etc are my fav medical drama's, funny how even though I can see mistakes and errors in the med drama's it doesn't really bother me too much  :twothumbs

Hmm... It's easy. Because we simply like it and feels lazy to complaining about every single little mistakes there and there.
Dr. Shinoda and Dr. Akimoto. I can imagining them in doctoral outfit right now.
Shinoda, troll and sadist as ever. But also have a good responsibility towards the patients. Dr. Akimoto's too.
Mocchi, Sasshi, and Haruna. Hope Haruna didn't spaced out so much. It would make patients in danger.

The one who saying it was Mariko, right?

“You are only saying that because Haruna is a part of the ICU staff.” Sayaka got up and headed out of the cafeteria, throwing away her empty coffee cup on the way out. Mariko just sat there starring out the window watching the rain pour out of the sky.

I smell some hints here... :grin:

O.o oops, I wonder how I missed that and no one told me about LOL. Thanks Caghaunt, yes it's Mariko saying that LOL.
This medical fic idea started out as an snsd fic I wrote a while ago but it didn't work out, wasn't able to control the hospital with
just the 9 of them so the story died off pretty soon, but the idea were still floating around so I took that apart, changed the whole
storyline and gave the story to AKB keke. I borrowed a couple lines from my old stuff so I guess I was too tired and missed the
mistake keke.  :twothumbs

Also you will have to figure out if the hints mean anything or not =P

Ah.. so that's what really happened.
Some old stuffs included in a new stuff sometimes can make us forget and thinks it wasn't a mistake.
I'm not hoping for any romantical things, though.

Playing with every characters...
Giving hints there and there. Even the rare/crack ones..
But not make all those hints any further!
I like that idea!
It would add some spices into the story, but keep the whole medical things undisturbed! 8D

it's been a while.... another one that I had to go searching for somewhere close to the bottom of the 2nd page   :P
Looking at the date it's been also a more then a month since I last updated this fic.... as mentioned in Impulse I'm sorry I got a little occupied with life.

Originally I planned to update this maybe a few days later but as per request from Haru I will update this today as well. I just can't get myself to refuse, not that  :twisted: This is another one of those longer updates just to make up for the lack of update in the month  :lol:

Living in Emergency: PAGE 3



           ~Life is precious but not fragile, one can only learn from experiences~



“So tired.” Sashihara complained as she plopped down beside Miichan. She splayed back on the couch before turning to face her buddy. When she got no acknowledgement, she switched her focus to the TV in front of them.


“You know, with the amount you eat I’m surprised you don’t end up like one of those obese patients we have in ICU.”


“I can’t help it that I’m hungry, I’ve been running around the hospital all morning dealing with calls from almost every ward. Some patients are really needy, having me sucking globs out of their lungs.” Miichan said while taking another bite out of the sandwich in her hands.


“You can talk about this stuff while eating?” Sashihara questioned.


“You can’t?” Miichan looked up faking a shocked expression, before she resumed her attack on the sandwich.


“I admit after working here, I could care less.” Sashihara replied while trying to reach for some cookies in front of Miichan, only to have her hand swatted away.


“Go get your own, these are mine! I got them from the nice nurses on the wards.”


“Probably annoyed them until they gave in just to get rid of you….” Sasshi whispered to herself.
 

“What did you say?!”


“Nothing, nothing… I transferred the lady from emergency to ICU already. Mariko-sama wanted to start the cooling procedures as early as possible.”


“I hope everything will be okay, I heard the granddaughter is having trouble adjusting to the fact,” a voice spoke up from behind the two resting RT’s.


“Ahh! Don’t scare us, Miyazaki!” The two on the couch shouted together.


“I told you to call me Myao or Miho! Plus it’s not my fault you didn’t hear the door open,” Myao made a face at her senior colleagues before dropping herself beside them on the couch. “So tired.”


“This hetare said the exact same words just a little while ago,”


“HEY!” Sasshi shouted but was unable to think of anything to defend herself.


“Is everything settled in ICU?” Miichan asked while she moved to clean her dirty dishes.


“We already have the cooling blankets running, the sedation is in, and ventilation has already been adjusted. Dr. Shinoda made sure to have everything ready so the transition would be quick,” Myao explained.



“I bet everyone was just afraid that if they don’t have everything ready they will be her trolling target for the next little while,” Miichan said with a chuckle.


“I don't think everyone’s like you, having a death wish to argue with the doctor,”



“Come on, break is over Gachapin, we still have lots of work to do,” Sasshi said as she dragged the other girl out of the break room.


"SASSHI! I still have dessert waiting for me!"

*Atsuko’s POV*


“Dr. Maeda, Please come to the trauma bay.” An announcement came overhead in the ER of Akibahara Hospital.


I quickly made my way across the unit to the trauma bay after excusing myself from another patient. Upon arrival, the curtains have already been pulled together, I brushed them aside to let myself in before closing it in one swift motion.


“What’s the story?” I asked one of the paramedics that brought the patient in. While watching the nursing staff in ER start hooking the patient up to the monitor and checking her vital signs, for heart rate (HR), blood pressure (BP), and temperature (Temp).


“Shoulder dislocation. 14 year old kid plays center forward on her soccer team, she was trying to get through the defense when she got body checked by some of the older girls on the opposing team. They fell on top of her in a pile, she’s suffering from a dislocated shoulder with noticeable imbalance but it doesn’t seem like she broke anything. There are a couple scratches and cuts on her arm and leg but those are minor from the fall.” I listen to one of the paramedics report the situation before moving closer to get a better look.


“How old did you say she is?” I asked after getting a good look at the girl’s face, she didn’t look one bit like a 14 year old but she give off a familiar feel.


“Sawako can you page the RT, tell them conscious sedation in 10min.” I shout through the curtains to where the ER unit clerk is sitting.


“Draw up some Lidocaine for the pain, I want some Propofol and Morphine ready for a conscious sedation, I am going to put the shoulder back in place.” I gave my orders to Yui before picking up the patient’s chart to see if there is any other history on the patient that I should know about.


"Matsui Jurina?" I asked.


"Hai!" the girl replied with a smile, her whole body beaming with energy.


"I am Dr. Maeda, your shoulder is dislocated from your fall earlier so I will have to put it back in place. I will give you some medication to make you sleepy for the procedure. You won't remember any of this. Do you understand?" I gave a quick explanation of what I have in planned.


"Yes, Rena tells me about these procedures often. She often talks about what she learns working with you and a Taka-midget in emergency. You sound like good doctors." Jurina spoke.


"You know Rena? Dr. Matsui Rena?"


"She is my sister," Jurina said, sticking her tongue out from behind that painfully happy smile.
 

"KAWAII!" Amina squealed as she paused from connecting Jurina to the monitors at bedside, tempting to walk over and run her hands over that perfect face.


“Amina, are the monitors connected yet? I want some vital signs." I quickly interrupted her before she scars Rena's sister with her harassments, "Is the RT here yet?” I asked no one in particular.


“Right behind you, wifey, been here for a while. You were too absorbed with your patient and nurse to notice.” I turned around and saw Miichan, one of the RT’s in the hospital, standing there in the corner, chewing on a granola bar. From where I am I can see a couple more sticking out of her scrub pockets.


“Why is it always you when I need an RT, all you do is stand there, eat and talk back.” I joked. Before signaling Yui for the sedation I need for the procedure, I started administering the Propofol after a little bit of Lidocaine through the IV line.


“Not my fault you work when I am scheduled to work, Dr. Maeda." she replied as she chewed, "Plus we are short on staff, so until the new hire starts, you’ll be seeing more of me."


"Jurina, you will feel a slight sting from the medication then start to feel sleepy okay." I told the girl, waiting for her to confirm with a nod before proceeding to give her a little bit of Morphine to dim the pain from the procedure.

 
"You always surprise me how you can talk and eat without choking." I said to Miichan, in reply to her astounding skills.


“How are you feeling?” I asked while shaking Jurina lightly, she just responded with a moan and a bit of twitching due to the pain. I pushed in a bit more morphine then moved to grab a hold of her arm and pulled it back in position. With 1 swift motion I heard a distincted popping sound indicating the shoulder has been settled back into its rightful place. I placed my hands around the shoulder joint, making sure it’s in properly and balanced before placing her arm on her stomach. Amina moved to place the arm in a sling while I went to wash my hands at the sink.


“She can go when she wakes up.” I said.


“That went smoothly. Not bad, Dr. Maeda.” Miichan spoke up from the exact same spot I saw her in before I started the procedure, chewing on what I believe is her 3rd granola bar.


“Thanks for your help RT.” I stated the obvious fact, with arms crossed not impressed.


“Well her oxygen is still 99%, she is still breathing, I’m just here in case she stops breathing or goes into a respiratory arrest. But she’s fine, so there isn't anything I can do for you. I’ll just stick around until she wakes up.” Miichan said while opening her 4th granola bar.


“How many of those do you have anyways?” I asked, kind of curious how many she brings around with her.


“A few, you want one?” she asked, taking another one out from her pocket.


“No thanks, I’m good.” I told her even though I kind of did, fighting the temptation. Last time I took the offer, Takamina made fun of me all shift.


“I know, that's why I offered.” She said without looking back.


“Hmmm, how about you come with me. We can grab a bite together, my place is pretty close from here hmmm~” The words came out as a mumble at first but were clear enough for all the present staff to hear. We all looked at Jurina, who continued to speak out loud.


“Those short skirts bring out your beautiful legs, mind if I touch them? *moans*” I look to Miichan with raised eyebrows, only to see her with her hands covering her mouth coughing as she almost choked on her granola bar from laughing. I pan the room to see the rest of the staff in similar states, trying to hold in their amused looks and laughter. I saw Akicha walk over to the bedside, trying to wake the poor girl before she lets everyone know what she’s dreaming about.


"Jurina, Jurina, wake up the procedure is over... Juri..." As Aki tried to shake the girl awake, somehow with her one good hand Jurina got a hold of Aki's scrubs and was able to pull her closer.


"Chu~kekeke" Jurina started giggling after catching the poor nurse by surprise. Bringing the rest of the emergency staff that caught the incidence in a new round of laughing fits.


“Seems like the Propofol is giving her a pretty good dream at the moment, I’ve gotta tell Rena about this when I see her.” I laughed before leaving the area.


Once I emerged from behind the curtains, I heard Sawako’s voice. “Dr. Maeda, I want to remind you that you will have a doctor trainee (Resident) following you starting your next set of shifts.”


“Thanks.” I waved back at her before leaving ER to grab a coffee from the cafeteria. And maybe some proper food too. Watching Miichan eat always make me hungry too.

In ICU

"My name is Kojima Haruna, I am the nurse taking care of your grandma for today." Haruna introduced herself as she lead the devastated Yuko to her grandma's bedside. She gave Yuko a silent pause and a couple seconds to take in all the machines and pumps around the bed before leading her straight to the side.

"Don't be afraid to hold her hands, let her know you are here." Haruna spoke softly as she picked up Yuko's hand and placed it on her grandma's.

"She might be a little cold since we are cooling her body slightly. Everything might seem confusing to you, but if there is any questions or concern, please don't hesitate to ask me." Trying to be as professional as possible, Haruna took the time to slowly explain and answer some of Yuko's questions.

"Thank you." Yuko said softly after Haruna took the time to slowly go through and answer each and every one of her questions.

"Oshima-san, Dr. Shinoda will like to have a family meeting with you tomorrow if it is possible for you. She would like to discuss and inform you of your grandma's conditions and build a care plan for her stay in the intensive care unit. There are some things you should know and need to know."

"That would be appreciated, Thank you again Kojima-san." Yuko said with a slight bow.

"I will let Dr. Shinoda know, and we will arrange a time for the team to sit down and talk with you. Oh, and please call me Haruna."

"Then call me Yuko."

Sasshi is the RT with Miichan and Myao lol and that is already scary enough HAHA.
Jurina won't be dreaming about Rena it would be someone else since Jurina, Rena and Sakiko will be sisters. But its okay it doesn't change the fact that Jurina has the "gotta catch em' all" goal to collect as many kisses as she can  :lol:

Yup Just to make it more scary you get a combo of the nurses you listed, suberu Takamina, Acchan and Sawako BWAHAHAHAHA

As Mayuyu is my oshimen, I'm gonna say I want Mayu first (though I'd actually like to see Mayu's story related to Yuki but you already have your story planned so I'm not going to interfere with that :grin:)!

Yes unfortunately Jurina and Rena are sisters in this one, but that doesn't stop Jurina from molesting... errr being friendly with everyone else when she does reappear in the story.
Haven't decided that yet.

Segius: Nah Jurina for sure won't be embarrasssed, its just sad that she doesn't remember collecting that kiss  :lol: I'll try to see if I can fit more of the RT team in the
story but I'm also trying to add in a bit of everything as well...(even if I really don't know the other bits as much as I know the RT stuff) I feel like sometimes I'm just making
things as I go HAHA. Let see hmmm Mayu first, I might be able to do that  :) I actually have no plans for this story, I just write as I go and go along as I write. All random and
spontaneous for most part.

            ~ Time is limited, so don’t waste it living someone else’s life~



“Good morning Oshima-san, I’m Dr. Shinoda, one of the intensive care doctors in this hospital. I’m glad to be able to sit here with you and talk to you about your grandma.” Mariko spoke slowly with one of her best smiles to maintain a positive atmosphere to start the meeting.

“As you already know, this is Kojima Haruna and she is the nurse assigned to care for your grandma today. Beside her is Oya Shizuka, the respiratory therapist that is scheduled in ICU for the next few days.” A quick introduction was made to make sure that Yuko is familiar with the present personnel before the important details were discussed.

“Good morning, please call me Yuko, Dr. Shinoda,” Yuko replied, “I thought Myao was taking care of my grandma?”

“Yes, Myao was in charge of your grandmother’s care for the last couple days, and she is not schedule to be back until tonight. It is a normal shift rotation that we go through, I will be here in ICU for the next couple days.” Shi-chan tried to simply explain their switch overs.

“Yuko, I assure you that our team of nurses and RT’s are the best and credited. Your grandma will be treated with respect and cared for as anyone and everyone else.” Mariko spoke up to reassure Yuko that her grandmother is receiving the best care available.

“Before I get into the details I want you to know that any decision of care we make today will not be limited by finance, if that is an issue please let us know and we will provide you with assistance. Do you understand? Finance is a concern BUT it should NOT influence or be used to direct care.” The experienced doctor made sure to emphasize her point, while maintaining eye contact, waiting for the nod before moving on.

“First I agree that your grandmother should be maintained and cared for with full medical management to the best of our abilities. What I would like to talk to you about is the full resuscitation in the event of another cardiac arrest.”

“What I mean by cardiac arrest and resuscitation is that in a situation where her heart stops we will try to restart her heart by physically pumping her heart with chest compressions,” a short pause of silences was given for Yuko to process the information as Dr. Shinoda illustrated what the compressions does with her hands, “so if this is the heart, each time we compress her chest it will squeeze the heart bringing blood to her body and attempt to restart these muscles.”

“For your grandmother, because of her age and her underlying complications and medical history, I believe that in the event where her heart stops again this process of pumping the heart to restart the muscles won’t be effective. All we will accomplish is possibly breaking her ribs when we start compressions.” another short paused was given as Mariko watch Yuko closely to make sure what is said is being understood.

“But, it worked didn’t it? Her heart came back when it stopped?” Yuko asked trying to understand.

“Yuko, you are aware that your grandmother had suffered from a heart attack that brought her into the hospital,” stated Mariko, after receiving a nod from Yuko she continued, “Are you aware of the other health issues your grandmother have?”

“I know she has diabetes and hypertension but everything happened so suddenly,” Yuko reasoned.

“Yes, your grandmother has diabetes that has not been properly controlled and her high blood pressure complicates her heart condition, this did not happen randomly.” Both points were reiterated to confirm what Yuko already know.

“There are 6 problems ongoing with your grandmother. First your grandmother’s uncontrolled diabetes which has caused her kidneys to not function. She only has one functioning kidney, but even that one is starting to fail. Her high blood pressure puts extra work on her heart and decreases the blood flow to her heart muscles. You are aware that she suffered from a heart attack leading in her heart muscles stopping for a significant amount of time, causing damage to those muscles. Finally you grandmother is still currently maintained on a breathing machine with a high amount of oxygen requirement and we are noticing a decrease in lung function.” Mariko continued to slowly explain to Yuko why further resuscitation will not be effective. For the next hour she slowly went through each body system to make sure Yuko understoodd the serious condition her grandmother is in. Throughout the full meeting, Haruna sat beside Yuko providing her with mental support and occasional assuring physical contacts and gestures to remind her that she isn’t alone.

*Elsewhere in ICU*


Today just isn’t a good day for Hiramatsu Kanako, ICU registered nurse with three years of experience. There was a sick call for the day and now ICU is short a nursing staff and on top of that, Haruna is stuck in a family meeting with Dr. Shinoda. Out of luck, Kanako receives 2 patients under her care, both not as sick as the rest of ICU’s population, but it doesn’t mean they are easy assignments. Her headache and frustration came from one of the 2 patients she is caring for. An elderly lady to be exact. This patient of her’s came in from a care home for elders; she was experiencing some chest pain and felt short of breaths last night. After connecting her to the monitor they found her heart in an atrial fibrillation rhythm. This is one of the common abnormal heart rhythms when a section of the heart muscles don’t contract properly and quivers instead, causing poor circulation to the body. For treatment, the doctor ordered for Amiodarone infusion, a drug given through the IV, that is only run through the ICU because it has effects on the heart rhythms. The last reason this patient is in ICU is because their hospital’s step-down unit is closed for renovation, so all patients are reassigned and trafficked into different units according to severity.

So recently here in ICU, the nurses are treated with the rare sight of an awake and talking patient, with demands. Earlier in the morning when Kanako first went to assess this patient, she already wanted to strangle her patient despite the smile she tried to keep on her face.


Earlier that day.


Kanako walked into the patients room and greeted the patient usual, whether they are awake or not. Hearing is said to be the last sense to go, so even if they are under sedation or not conscious, the patient might still hear what is said around them.

“Good morning, my name is Kanako, I will be your nurse for the day,” she said and gave her patient a bright smile, just like how they learned in school. A smile is the best way to establish a rapport with their patients. If the patient experiences a positive response, they are more willing to compile with their treatment.

“Go away, I don’t want students taking care of me! Go get someone older and more experienced!” The lady said in a rude tone.

“Ms Sadako, I assure you I am experienced as a nurse, I have been doing this for a few years now.” Kanako tried to reassure her patient, already feeling offended with what was directed towards her.

“Don’t lie, you people always say that so you can get more practice. I want a different nurse. Go back to school and practice for a couple more years before you come back.”

“Look, I graduated nursing school, passed my registration exam and have been working as an ICU nurse for 3 years now. You are under my care, we don’t have any other nurse.” Kanako said, her voice noticeably laced with a hint of anger.

Kanako is beyond pissed with this unreasonable patient of her. This isn’t the first time someone has mistaken her age because of her voice and appearance, neglecting her accreditation and certification.  Most patients and patient’s family that come to visit also think she is a student because of her appearance. Worse, some even think it’s “bring your kids to work day” and she is here with her parents to see how work is for them. In the back of her mind she can already imagine Ms. Sadako crawling out of bed and on the ground towards her with a list of demands. Definitely a nightmare, Kanako starts praying to god that she won't be seeing that in her sleep.


Present time

“NURSE!!! NURSE!!!” The shout resonated through the air of ICU. In the background, the tapping of plastic on the bed rails could be heard.

With a sigh, Kanako got up from her spot and headed towards the devil herself, hoping to cease the assaults that dare disrupt the peaceful atmosphere of ICU.
As soon as Kanako entered the room she was bombarded with a string of demands.

“Nurse, I’m thirsty go get me some water. I’m uncomfortable in this position, I need to be repositioned. And I’m bored, go find me some magazines or books to read. My hands are itchy! Can you take out this line?!” the patient listed while pointing at her IV.

“Let me get some help, then I will reposition you in bed, after that I can bring you some water and see if I can find you some magazines, the IV has to stay in.” Kanako said with the most pleasant tone she can manage.

After boosting and repositioning the lady, Kanako went to gather some magazines and water for her majesty. Today is really not a good day for Kanako, registered nurse with 3 years of experience. The minute she placed the items in her hand, she received new orders as if she just signed a slave contract.

“I’m hungry, order me some pizza, I want deluxe with thin crust.” The lady demand again.

“I’m sorry, this is a hospital not a hotel, we don’t do room service and we don’t do take out either. You are not allowed to eat outside food, your dietician has left strict orders for what you can have while you are in the hospital.” Kanako is more than fed up at this point. All day, she has been doing ridiculous tasks for this patient of hers. She is a nurse, not a servant. Maybe she should’ve just wore a maid outfit to work just to serve her queen in her royal bed.

“Those things on the tray aren’t food, I want real food, I need to eat or I will die.” The elderly lady argued.

“I’m sorry, that is all you are allowed to eat. When you get better and leave the hospital you can eat anything you want.”

“How am I going to leave the hospital if you horrible people are trying to starve me and not give me food?!?” said the irrational lady.

At this point, Kanako was ready to surrender. A part of her had the urge to go right up to the patient and strangle her to end the misery. Why did she even agree to do this shift trade? If only she never decided to help out another fellow nurse and just stick to her own working schedule, she wouldn’t have to deal with this.

“Thanks for coming, it was nice meeting you. I know you haven’t been working for a year, but your previous answers to my questions has shown otherwise. I’m quite pleased with your knowledge level,” spoke the manager of ICU.

“I would also like to ask a couple more personal questions to get to know you a bit more. Is that okay?” The manager waited for the sign to go ahead before continuing her list of interview questions.

“If you are not comfortable with the question, you don’t have to answer.
Now, why did you want to become a nurse and what was the reason for you to leave for a year of travelling after you graduated instead of diving into work like most graduates?”

“When I was young, I was diagnosed with cancer. I spend a lot of time in the hospital, this hospital to be exact. Under the care of Dr. Akimoto. I still remember he used to allow me to call him Yasusu.” Masuda Yuka spoke personally about her past that made her so passionate about her career and her life.

“I don’t remember a lot during my hospitalization, other than the fact that I was consistently there and had IV’s and injections everyday. I continued my treatment even after I was allowed to leave the hospital,” she paused to take a breath in, “I still don’t know why the cancer was able to completely heal but to me it was an experience that impacts me greatly, something that made me stronger. I almost died once before, but had miraculously recovered so I took a year off to travel and see the world while helping others that are in need. After a year, I believe it’s time I return to my home and start where I once was a patient, but this time I will be the one providing the care. It is my dream,” Her story ends here and out of the corner of her eyes Yuka swears she saw glitters of tears in the eyes of her interviewer.

“I am glad Dr. Akimoto personally referred you to our ICU but I will still have to ask you to wait for our call. There are other applicants that we have to consider and some still have yet to come for their interview. We will notify you by the end of the week, if you are hired.”

Yuka stood up from her chair, bowed then shook hands with the ICU manager of Akihabara hospital’s then proceed to walk out of the office. Once she closed the door on her way out, she let her breath that she didn’t notice she was holding, looking back at the plate written on the office door, Eguchi Aimi. Yuka never thought she could be so blown away by another female, but today proved her wrong. The minute Yuka stepped into the room she was taken back with what she saw. The young beautiful yet plastic looking manager of ICU, standing in front of a mirror at one side of the office, admiring herself. Not what you would imagine from a manager of a hospital’s ICU, one would think when the door opened that you would see a middle aged person in a suit sitting at the desk in front of a computer dealing with paper work. The smile on her face gives off one of those shining but fake feelings. During the time of their interview the perfect manager appeared to be very nice and friendly but Yuka couldn’t stop herself from getting this feel of evilness that lurks in the dark corners watching her every movement. Is this the reason why Sayaka had told her to be careful of the doll?

Making her way out of the hospital, Yuka headed towards a cafe near by the hospital where she had planned to meet up with Sayaka after her interview, to catch up on their old days. She still has to thank Sayaka and Uncle Yasusu for giving her some reference to apply to the hospital. It would look bad on her part for a new graduate to disappear for a year after graduation then suddenly appear out of nowhere with no references.

Normally I guess people would like to look younger then their real age but I guess in this case it does get really annoying. It actually still happens to me .... and I've been working for a few years now....

Living in Emergency: PAGE 5


*Sigh*

 “Where is she? She said she would be here.” said a disappointed Sae, while twirling her phone in her hand, occasionally checking the time on the screen.

 It’s already been an hour since her best friend was supposed to be here in that vacant seat across from her. Every time she looks up to see that empty chair it adds to her frustration. The waitress had already come by a couple times to ask if she would like anything to drink; each time Sae would refuse and just ask to have her water refilled. Despite being hungry, Sae really wanted to wait for her twin tower to be here so she continued to wait, ignoring her protesting stomach. The two of them have been buddies and bestfriends since back in their kiddie days; from what they could remember, it was always the two of them. The other kids felt intimidated by the two's height so they tend to shy away or make fun of their differences. Deep down inside Sae knew her twin wouldn’t ditch her at the restaurant all alone unless she has a good reason. Must be tied up at work. Hopefully everything is okay.

At the other side of the restaurant, another girl sat quietly at her table, seeming to wait for someone as well. She checked her watch a while ago before settling to observe her surroundings. It was a decent looking restaurant that emitted a pleasant and cozy atmosphere. The lights were set to a dim glow, but you could still pick out the designs on the wall if you looked closely. What caught her eye was the shiny black grand piano on the restaurant’s stage, set to automatically play an all too familiar beethoven piece. While enjoying the lovely music, she noticed another girl sitting alone at a table, constantly checking the time on her phone. Feeling sympathetic, Mayu suddenly had the urge to walk over there to keep her company.

“Sorry I’m late, I got caught in traffic, did you have to wait long?” came a voice, jolting Mayu back to real time.

She turned her head back to the seat in front of her just in time to accept the flowers and catch her late dinner date seat himself across from her. It was another one of her mother’s attempt to set her up so she won't end up old, wrinkly and lonely after she becomes a dedicated hospital workaholic. Being a doctor comes with a lot of responsibilities, the amount of time invested to care for each patient is really consuming. If you are not dedicated and motivated to help those that come seeking for assistance then you would be bound to fail.

“That's okay. I’ve only been here around 10 minutes. I didn’t know it was you that I was meeting for dinner tonight.” said a slightly surprised Mayu as she placed the flowers in one of the chairs at the side.

“You didn’t know? I tried calling your cell but you didn’t pick up so I called your home and auntie picked up. She said she would let you know about dinner. Congratulations in getting placed in Akihabara Hospital for your residency! I heard that was your first choice. Here, I have a present for you.” He took out a small wrapped present and slid it across the table to Mayu.”

“Thanks, but you didn’t have to get me anything.” Mayu carefully unwrapped the present and opened the box to see a beautiful necklace, with a silver ring hanging on it. Mayu stared at the ring for a bit before pushing it back across the table.

“Sorry, but I can not accept this gift from you.” Mayu said.

“Why not? Why can’t you give me a chance? Let me show you how much I care. Let me be the one that you can turn to when you need someone. Please believe me.” He said as he slowly moved his hand over to rest on top of her’s.

With his other hand he presented the necklace to her again, hoping she would reconsider. But Mayu stood strong to her first answer. Looking straight into his eyes, she removed her hand from his hold and pushed the necklace back across the table. Just as she was about to answer him with harsh words, she was interrupted by the sound of glass and plates hitting the ground.

“HELP!!! HELP!!!” The screaming of a woman could be heard throughout the restaurant. A small crowd was slowly beginning to form around a table a couple meters away. Mayu immediately got out of her chair when she spotted an elderly male holding his throat with one hand while waving frantically with his other, unable to make a sound. A few quick steps took her to where the choking man was, slightly bluish tint appearing at his lips and ear tips.

“Let me help you, I’m a doctor. Someone call for an ambulance!” Mayu quickly moved next to the choking man who quickly nodded his consent after hearing Mayu is a doctor.

Mayu repositioned her arms in front of the man's abdomen, right below the diaphragm, and performed a perfect abdominal thrust from behind the choking man. With both hands performing a hard compression into an upward motion, Mayu attempted to dislodge the piece of steak that migrated into the airway, making it impossible for the man to breath. After a couple tries, Mayu was still unable to dislodge the piece of meat that prevented the elderly and slightly overweight man from taking a long delayed breath. His face began to turn blue, to the point of becoming cyanotic. Just as Mayu readied herself to try again, a hand grabbed hold of hers and pulled the man out of her reach.

“Let me try, I’m a Respiratory Therapist.” Sae spoke up as she got into position for abdominal thrusts.

Mayu turned to see the taller girl she was watching earlier. With one smooth motion she executed the maneuver with such a force that you could almost see the man being lifted off the ground. After her second try a coughing sound could be heard from the man, but he soon became limp and unconscious in the girls arms. She quickly laid the man down on the ground, pulled out a pair of gloves from her bag, then checked his mouth for any signs of that piece of steak. After doing a finger sweep to double check, she repositioned his head to see if that would open up the airways.

“Is he breathing?” Mayu asked

“No, I think the piece of meat is still in there.” Sae replied. Mayu immediately started doing compressions hoping that the pressure in the lungs can be enough to dislodge that stubborn piece of meat and allow some air to pass through.

“Thats about 2 min, check for pulse while I check for the steak.” Sae said as she went straight to opening the mouth and looking for that piece of steak.

“I SEE IT!” said an excited Sae, she quickly did a finger sweep to removed the guilty piece of half chewed up beef. All the while Mayu rechecked for a pulse and breathing.

“He doesn’t have a pulse, I am continuing compressions.” Mayu said automatically.

“Yes, doctor.”

Sae repositioned his head to make sure the airway is clear. In the old days, they would be taught to give mouth to mouth in this kind of situation, but in the rise of a new era mouth to mouth is not necessary anymore. The two work diligently together, complimenting each other like they have always been working on the same team. No delays were made, their movements flowing with one another’s.

After a few minutes, ambulance sirens can be heard, followed by the clattering of the stretcher and sounds of rolling wheels getting louder as the clock ticked away. The paramedic crew quickly took over the resuscitation, one continued with compressions while another grabbed a breathing tube from their equipment bag. Normally in a situation like this, Mayu would be happy to intubate (process of inserting the breathing tube into the lungs) the person but her arms are tired from all the compression and she didn’t want to delay care as time is now crucial for the choking victim.

Once the breathing tube wass in, they connected a bagger to the tube and squeezed oxygen into the lungs. Mayu assisted in connecting the the AED (automated external defibrillator) after notifying the crew she is a doctor, Sae took over the airway part while the paramedics concentrated on compressing the heart to circulate the blood.

“Okay AED is charged and ready,” Mayu announced “I am clear, you are clear, everyone is clear, Shocking.” She declared as she looked to make sure she and everyone is clear, before pressing the shock button.

“Doctor we have a rhythm, and a pulse.” One of the paramedics stated as he checked the patient for pulse. “Lets move him to the stretcher and take him to the closest hospital.”

“I’ll come with you.” Mayu stated.

“I’m coming along too, I can watch the airway and breathing tube.” Sae added

“Glad to have you along. Let’s go. The closest hospital is Akihabara.” said the paramedic.

The minute everyone got loaded into the ambulance, the sirens could be heard sounding through the streets.

“I haven’t had a chance to introduce myself earlier, my name is Shimada Haruka.” said one of the paramedics sitting in the back with Mayu and Sae. “The one driving is Nito Moeno,” she continued while waving her hand towards the driver in the front.
“Hi, nice to meet you.” Moeno said while keeping her eyes on the road.

“My name is Watanabe Mayu, you can call me Mayu or Dr. Watanabe. I’ll be doing my residency at Akihabara Hospital.” Mayu introduced herself.

“Really? What a coincidence, I’m going to be starting at Akihabara Hospital too! Miyazawa Sae, I’m a Registered Respiratory Therapist," Sae said while making sure to maintain a steady breathing rate, pushing oxygen into the patients lungs with the bagger.

When the ambulance arrived at the emergency entrance of AKB hospital, they exitted the vehicle and pushed the patient into the trauma bay, where nurses and doctors were already waiting for them. As they transfer redthe patient over to their stretcher, Mayu approached the doctors to give them a report as to what happened at the scene.

“He choked on a piece of steak that occluded his airways. After getting consent, I attempted to do a couple abdominal thrusts but was unsuccessful. The RT over there took over and tried a couple abdominal thrusts, she was able to get the patient to cough a little bit but he went unconscious soon after. We started CPR immediately and was able to dislodge the piece of steak but he had no pulse so we continued CPR for another 5min before ambulance arrived. They intubated and stabilized the airway before transporting the patient here. No other issue on route. Patient was manually ventilated by the RT.” Mayu quickly and precisely reported to the emergency doctor before stepping out of the way for him to assess the patient.

“Sasshi, I want ABG’s after patient settles on the ventilation. We will transfer the patient to ICU after we stabilize and get a baseline.  Kumi, I want a full set of blood work done and a 12 lead ECG. Shawako, call radiology for a stat chest x-ray, I want to make sure the breathing tube is in the correct position. Mizuki, can you start an IV on the patient?”

“Ok, Dr. Aamin.” chorused through the trauma bay as everyone started on their task after Dr. Maeda Ami gave out her orders. Although Ami is still only a resident, she is well known by the staff as one of their ace doctors, trained personally by her sister Maeda Atsuko and Dr. Takahashi. She has been approved by the head of the department Dr. Takahashi to have the ability to self manage cases without requiring supervision.

Sashihara took over for Sae after setting up and connecting the patient to a ventilator. While Yagami Kumi, one of the laboratory technicians started gathering her tubes to collect blood for lab testing. Kuwabara Mizuki, another one of emergency's well known nurses, went to one arm, looking for a vein to start the intravenous line. At the same time Sawako is already on the phone talking with radiology for that chest x-ray the doctor wanted immediately.

Surprising the emergency team, Dr. S. Akimoto came walking into the unit to assess this ventilated patient who will need ICU's higher level of care shortly. It was her way of speeding up the process so the patient doesn't have to stall in emergency waiting. During the whole time she was assessing the patient, Sayaka could feel a pair of eyes glaring at her back. When she finished writing down some medication orders and admission to ICU orders, she turned around to come face to face with an upset Sae.

“Sorry Sae, I was caught up with a patient earlier. I didn’t even notice the time. I’ll make it up to you another time... Come on, you forgive your twin right?” Sayaka tried to worm her way out of trouble.

*Sigh* “You could’ve at least called to let me know. Then I would’ve actually ordered and ate. You owe me two dinners for anything I want, no objection.” Sae said. “Anyways, I guess you’ll be awhile. I’ll head home first.”

Once the patient was handed over to the hospital staff and stabilized, Mayu dug into her bag to find her cellphone so she could call a cab and let her parents know that she will be home late. She found her phone with no problem but she also found something that she didn’t expect to see: the small box containing the same ring and necklace from earlier. Seichi must have put it in her bag when he passed her while she was heading out to the ambulance with the patient. With a sigh, Mayu stuffed the box back into her bag. She will just have to deal with him another time.

"Dr. Watanabe, do you need a ride?" Sae asked as she watched the young resident search up for the number to a taxi company.

"Didn't you come in the ambulance with me? Where did your car come from?" Mayu asked, surprised by the invitation and the logic that she is already trying to solve.

"Oh, I just borrowed my friends car. She said she probably won't need it tonight seeing how this patient just came in." Sae replied while pointing at the busy doctor behind her in lieutenant general mode, giving verbal commands to the available staffs.

"Thanks for the ride then, Miyazawa-san," Mayu answered with a smile after a moment. It would cost her money and time to call and wait for the taxi.

"Call me Sae. Say, are you hungry? I don't think you've had dinner yet." the genki girl asked.

"How did you know? Were you watching me?" Mayu asked.

"Errrm.... I.... well...." Shocked from the sudden questions, words didn't seem to want to process properly for Sae.

"Lets go! I'm starving," The young resident said with a teasing smile as she started to head out the door. Oh, and please call me Mayu when I'm not at work,"

"Ok, so what do you want to eat?" The revived genking asked as she sped up her pace to match the young doctor.

sakura_drop_: Yes blame Sayaka for the SaeMayu that is happening at the end, its because she didn't show up and left her twin tower there waiting all alone and starving! Plus it really takes a lot of energy to run through a full resuscitation with compressions so they must be so hungry that food is the most important!

Yes, it feels like something's missing if Yukirin isn't there with Mayuyu! Don't make us wait too long for Yuki's grand entrance! For some reason, I'm imagining a Dr. Watanabe and Nurse Kashiwagi combination. It'd provide a nice twist to their relationship O0

Dawww I had to go digging for this lol found it on page 6, it shows how long it took me to update. Immo's new speed of ants... or worst HAHA. Should've asked someone to just bump it for me but then people are busy too.

First to answer some comments from ancient times...

seigus: Hmmm Dr. Watanabe and Nurse Kashiwagi... that could be an option but I wonder when Yuki is really going to appear? This story will go on for quite I'm not even sure how I can end it, we are talking about hospital, doctors, nurses, and the 48 family... they can go on forever  :twisted: So much that can go on and so much to talk about really...

Megumi: I wonder would Sae and Mayu pair together for this or do I have other plans  :twisted: bwahahahahaha options and options. Yea I'm trying to integrate a lot of real case scenarios into this but then of course with a lot of edits and modifications to make it fit well and more interesting and less plain medical to put people to sleep haha. Don't worry I just need to find the perfect chance for Mayu to dump the guy.

*Intensive Care Unit - ICU*


“Yuko, are you ready?” Haruna asked from beside Yuko. Today is the day they have decided to finally terminally wean and extubate her grandma.

They met again in another family meeting to discuss her grandma’s condition, and the result of the neurology consults. Mrs. Oshima has been assessed and declared legally brain dead by two different doctors as per her request to make sure there are no errors in the overall assessment.

Dr. Shinoda and Dr. Akimoto were kind enough to take some time in explaining to Yuko what it means to be brain dead given that her grandma’s heart is still beating: an irreversible end of all brain activity due to the loss of brain oxygenation. So because her grandma suffered a major heart attack, the amount of time that her brain went without oxygen caused all functions to stop. Her body is only maintained by the technology of a respiratory ventilator and a temporary pacemaker in the heart.

As blunt as it may sound, their words were nothing short of true. The machines are just keeping her grandma’s body alive, and with all due respect, it is not the way it should be left. Yuko understood what they were saying but she needed a little more time, so together they agreed on a day, a time so that Yuko can have a bit more time alone with her closest and only relative left.

Walking up to the bedside with the support from the day nurse Haruna, Yuko bid her last farewell to the one that raised and took care of her.

“Bye grandma, rest well. Don’t worry about me, I can take care of myself now. Be at peace. Everything will be alright. I will miss you but I will never forget you and your love.” Yuko whispered into her grandma’s ears even though she couldn’t hear her anymore. With a last kiss on the forehead, Yuko stepped back and took a deep breath in, fighting the threatening tears.

“Okay I’m ready,” she gave the final signal and watched the respiratory therapists move over to the ventilator. One of her only wish is to be able to stay and watch her grandma go.

“Sae, do you want me to do this?” Miichan asked, seeing how today is only Sae’s first week of orientation shift and it seemed a little cruel to force her into the most dreaded task of withdrawing care. Given that it is still one of their responsibilities, Miichan liked to give new staff a choice to ease them into this part of the job.

“I’m okay, I can do it.” Sae said with confidence, not because she liked to do it, but because she understood the reason why it needs to be done and its inevitability. When all methods of preserving life has been exhausted, it comes to the point where end of life and comfort is more important, and continuing to maintain an already dead person is unethical and immoral. Sae believes that this is an important part of their job, to be able to preserve where appropriate but to also understand when to allow patients to rest in peace. Health care workers really aren’t cold blooded nor do they get used to seeing people die, they just understand the process and when it is time to let go.

“May you rest in peace, Mrs. Oshima.” With those last words, Sae turned off the breathing machine.

She then quickly moved over to cut the ties from the breathing tube, removing them from the lungs along with the oral gastric tube that was inserted into the stomach. Miichan was ready on the other side with a damp towel to clean Mrs. Oshima’s face one last time so she will look her best before she leaves. Kanako moved over to the monitor to turn it off, preventing it from seeking attention from anyone present; it will not be the focus of this important moment.

“Grandma!!!” Yuko sobbed into the arms of Haruna as her body lost its strength to stand. This is it. From now on she will be alone.

Haruna just stood there holding Yuko, letting her properly mourn while helping the girl dry her tears every so often. It really is a sad case and there isn’t anything else Haruna can do for the girl except hold her tight and let her cry. Everything else has already been arranged with the help of Kayo Noro, their in hospital’s ICU social worker. Noro has already help Yuko planned the funeral according to Yuko’s wishes prior to today so Yuko won’t have anything else to worry about at the moment. This is not normally the responsibility of social workers but Noro usually goes above and beyond to help family cope and assist them as much as she can.


*Some moments later*


“I’ll be okay, don’t worry about me,” Yuko said to Haruna as she stepped out of ICU into the family room, “I’ll be going now. Thank you for caring for my grandma.”

With that, Yuko walked out of the ICU and away from the place that she spent most of her last week.

“I hope she’ll be okay.” Natsumi said to Haruna as they watched Yuko leave.

“Matsubara! Your patient has been ringing for you for the last minute!” Miichan spoke with her outside voice which travels quite well across the unit to the ears of the two nurses.

“I’m coming, I’m coming,” Natsumi replied, sticking her tongue out at the annoying RT, who returned the favor.

“Kids, stop playing around. We have work to do!” Haruna reminded them. Of course the words coming from her mouth caused everyone else to stop what they are doing to check they weren’t just hearing things.

“What?” asked the confused Haruna.

“Nothing, let’s get back to work.” Dr. Shinoda spoke. Being the authoritative figure in ICU, her words were basically orders.

*Emergency Unit - ER*


“Arrrgghhh. I am so tired.” Dr. Takahashi spoked while trying to stretch her arms out to loosen those tight knots in her shoulder muscles.

“That was a pretty busy shift, patients just won’t stop coming.” Dr. Maeda replied as she started to pack up her stuff, ready to leave.

“Two more minutes and we’ll be free souls until tomorrow.” The two mature doctors high fived each other while they watched the second hand tick away to their freedom.

“Thanks for setting a good example for new staff.” A voice spoke from behind the two, causing them to slightly jump from their chairs. Being the unfortunate one, Dr. Takahashi ended up on the ground when her chair rolled a little off center.

Dr. Maeda tried her hardest to hold in her laughter as she watch her best friend rub her sore ass after getting off the ground. “Well Dr. Matsui, we are officially off duty since 5 seconds ago, so we’ll be leaving emergency and our new charge in your care.” Dr. Maeda or Atsuko answered, moving to take off her white gown.

“Come one Acchan, let’s get out of here before Rena starts another lecture.” Takamina said already halfway out the doors.

“That’s Dr. Matsui to you!” The sole emergency doctor of the day spoke firmly.

Dr. Matsui could only sigh, watching her two grown up collegues run out of the emergency doors. She knows that this is just the way they are, it’s their way of preserving humanity and relaxing after a stressful day. They become brats.... From the way they act to the way they address each other, everything changes. It’s like they have a hidden personality that appears the second they are off work and refuses to even answer if you call them doctors. But even so, everyone knows that they are still very reliable and can be professional even during their days off... they just choose not to be most of the time.

Rena lifted her hand to massage the bridge of her nose, hoping to relieve herself of the phantom headache threatening to appear. Finally deciding to ignore the two kids, she retreated back into the office to overlook the charting and assessment from their new resident. After reading the evaluations left by the other two doctors, they agreed that with some training and experience Dr. Watanabe will definitely be an asset to their department. In the near future, they hoped to have her self manage cases like Aamin.

*The Parking lot*


“Acchan! Hurry up, we are wasting precious minutes of our lives still stuck here in the parking lot right outside the emergency doors!” Takamina whined from the shotgun seat watching the second hand tick away on the clock.

“Shut up, if you complain again, I’ll make you walk home yourself.” Acchan said while she slowly adjusted her seat, making sure to take her time just to watch her impatient friend sit restlessly in the front seat.

Acchan took a couple more minutes to fix her mirrors before finally starting up the car, checked to make sure her path was clear, before pulling out from her stall. She got ready to book it and finally head home for a nice long shower before-

“Wait! STOP!” Takamina’s voice suddenly pierced through the air causing the car to come screeching to a halt.

When Acchan turned to investigate the sudden outburst, all she found was an empty passenger seat with the car door left ajar, the little body nowhere to be seen. Quickly, she looked up to scan the immediate area for the mini body finally finding it crouch down in the corner beside the hospital building. She took one quick looked around, checking to make sure there is no one else around, before getting out of the car and leaving it in the middle of the driveway. She was slightly, juuuust slightly concerned about the sudden actions by her best buddy who practically jumped out of the moving vehicle. As she moved closer, Acchan found another person sitting there on the ground with both legs hugged tight to the body, head down with long strands of hair flowing down the arms and covering the face. The sound of quiet sobbing could be heard coming from the individual which Takamina seem to be concerned and hovering over.

“Yuko, are you okay?” Takamina spoke softly while reaching out to the crying girl. Upon hearing the name Acchan, quickly moved to the their side.

“Takamina?” Yuko slowly lifted her head in the direction of the sound, visions blurred from her tears as she could no longer hold them in, knowing that she is all alone now.

“Shhhhhh. It will be okay.” Takamina pulled Yuko into a hug and let her cry on her shoulders. She had heard from Mariko that today was the day they decided to stop life support.

“I already miss her.” Yuko said in between crying and trying to breath.

Acchan could only watch at their side, feeling somewhat helpless as she is unable to do anything while Yuko cries her heart out. They knew each other back in their school days but Acchan wasn’t as close to Yuko as Takamina is, so it felt a little awkward to butt in during these crucial moments.

“She’s at a better place now and she will always be there watching over you,” Takamina said while she held the crying Yuko until the girl finally calm down. Eventually, the tiredness and emotional drain from the day’s events led the crying Yuko to fall asleep in Takamina’s arms.

“Acchan.” Takamina started, then paused to look for the exact words for her to finish her sentence.

“Hmmm?” Acchan waited patiently for Takamina to finish her question, already knowing exactly what she wanted to ask. They have known each other since they were little kids, afterall.

“We still have that extra room right?”

Acchan only nodded to the question as Takamina continued after seeing the confirmation.

“Do you think we can...”

“Come on little one, let’s get you two home and dried. We don’t want to have more sick little kids running around.” Acchan spoke, not waiting for Takamina to finish after feeling small droplets of rain hit her face. She walked over to the car, opened the door to the back seats and signalled for Takamina to carry the sleeping Yuko over.

“How did you know what I was going to ask?” The confused doctor asked, still on the ground holding the sleeping girl.

“Come on, I’ve known you since you were this tall.” Acchan purposely placed her hands at at her friend’s nose height, just to lighten up the mood before getting into the car. “Hurry, you don’t want Yuko to get sick too right? Let’s get home.”

*Late night in ICU*


A peaceful night in the hospital where patients were suppose to be sleeping to the soft beep of IV pumps, distant humming of life sustaining ventilators. This is when nurses sit quietly at their station with the sounds of flipping pages while they going over the medications and charts of their respective assignments.

Of course being in ICU, quiet has always been a forbidden word. This “Q” word is believed to be a curse, a jinx to steady working days or nights just like tonight. Soon enough, the usual peace got disturbed by the screaming and shouting from one of their rare awake patients, complaining of... not being giving any food. The young gentleman was admitted to ICU for close monitoring due to questionable seizuring activities, possibly caused by some unknown drug and substances. He is said to be healthy with only a history of bipolarity and depression.

“Where’s my food!?”

“Please calm down, we are getting you something to eat.” Rie spoke clearly, hoping to calm the lad before he does something dangerous.

“Here is some PB + Jelly Toast and juice.” Yuka came in with some snacks to buy them time while Kanako was outside on the phone with the patient’s family.

“That’s not food! I want to call my dad, he will bring me food.”

“We will call your dad for you.” Yuka told him.

“Lies! You people want to starve me, I haven’t eaten since this morning!”

“Please, someone is already on the phone with your father.” The charge nurse, Takayanagi Akane, came in to see if there is anything she can help with.

“LIES! Why won’t you give me anything! I’ll SUE YOU!” the patient said while trying to kick and punch at the nurses but failing due to his unsteady steps.

After standing there watching the scene play out, the lone RT in ICU decided it was time to speak up. Sasshi turned over to Kanako and said “Call security,”

“Mister, can you please get back in bed? NO, don’t pull on your  IV’s,” Rie shouted, worried that if they lose the IV access they won’t be able to give him any medication when needed and more importantly he might develop hematoma’s if not properly looked at.

“Shut UP! you are all Liars! LIARS!!! I just want some food!”

“We have some toast here for you until your dad gets here, can you please get back into bed?” Yuka now tried the nice approach to see if maybe, just maybe he will listen.

At this time the security team comes running into ICU. Between the couple of them, they successfully directed the patient back into bed before placing hard restraints. This did little to calm his nerves and he continued to scream and shout, pulling at his IV’s and tugging at this now restrained arms and legs. Seeing how he is getting a bit out of control, they decided to add the chest belt to hold him down. It’s the only way they can keep him in his bed and prevent him from harming the staff. During the time of wrestling him back into bed, his dad arrived and was escorted in to see if he would be able to calm the kid down. But even his dad wasn’t able to work his magic anymore. The young patient just seemed to get more and more restless, agitated and aggressive towards everyone.

“Dr. Akimoto said we can give him some Ativan to see if it will work.” Kanako shouted from outside after getting off the phone with the doctor.

“He pulled out his IV so we don’t have any access to give him the medication,” Rie pointed out.

“We will hold him down for you, just give it to him.” Nakanishi Yuka said from the bedside while she put her security training to use and placed a firm grip on the patient, with her partner Yamamoto Sakaya on the other side doing exactly the same thing.

*a few mins later*

Patient has now recieved a couple dose of Ativan with no effect on him. He continued to be very aggressive and began swearing at everyone present.

“You F*******, A******, Stupid asians! Taking up all the jobs in the country, I’ll sue you all! I just want food!”

“Your dad is here with food for you but you have to calm down first,” Masuda Yuka said from outside the room, her voice lacking the life she started her shift with. It’s only her first shift after orientation and already she is blessed with such a lovely night.

“I am calm you F*******, B*****. Let me go!”

“Let’s move him to the corner room so he won’t be as disruptive” Akane suggested after looking at some of the other patient and family members that are at the bedside with their sick love ones.

“I need to go pee, LET ME GO! I need to go do the bathroom,” the patient continue to shout as his bed got wheeled over to the room in the corner.

“You can use the urinal, you can’t get up to the bathroom.” Sasshi told the patient as she checked to make sure all the straps on the restraints are secured, she doesn’t wanna get murdered for provoking the patient.

“I can’t go like this, I need to take a shit!,”

“You will have to use the bedpan.” Rie said as she sat there, trying not to laugh at the situation. It’s not nice to laugh at the poor guy but it was kinda funny at the moment and getting quite entertaining.

“What bedpan, I can’t use the bedpan, it’s against my religion! I can’t shit upside down!!!!”

With that statement, both security guards doubled over and started laughing so hard that it began to hurt their stomachs. The nurses faired a little bit better as they tried to hide their face behind their charts while sounds of giggling through to everyone’s ears.

“You come back here and unlock me! I have rights, and I’ll sue you! You will all lose your job and end up on the streets!”

“This isn’t a hospital! I want to see your credentials! You are all illegal! ILLEGAL!! THIS ISN”T A HOSPITAL!”

“LET ME GO!!!”

“Think he will break the bed?” Sasshi asked.

*CLONK!* *BANG*

“Maybe...” Rie said and with one quick moment she closed the sliding door to the room to get some barrier to muffle out the screaming a little.

“Does this kind of stuff happen often?” Yuka asked.

“Of course not. These are just rare cases.” Akane tried to assure their new staff.

“What are you talking about?” Sasshi suddenly said, “What about the patient we had that ripped apart his own central line and was trying to shoot us with the blood that pours out from it?”

“..... Sasshi.”

“Or the  suicidal patient that threatened you with a plastic fork because she was convinced we were aliens and out to ruin her plans.” Sasshi continued, obvious to the looks Akane was giving her.

“Oh oh, then there was the one that kept trying to bribe us with food from his lunch tray, hoping we can help him escape from the hospital.”

“Sasshi,” Rie placed a hand on Sasshi’s shoulders hoping she will catch on and stop before Akane rage flips her to the ground.

“Then there was...” Sasshi paused after finally noticing the deadly glare she was getting from the charge nurse, “Ah look at the time, it’s time for my break. I’ll call Shi-chan to come in here and cover me.” Sasshi said quickly before running out of the ICU.

“Don’t listen to her, those are just only a few incidents we had here since Sasshi has started working. That’s like a handful out of all the cases we deal with daily.” Rie told Yuka with a smile while she watched Kanako try to calm their head nurse so that Sasshi could live another day.

I might start running out... gotta preserve them more and not kill everyone off I guess haha  :twisted: I wonder what will happen next. ahhhh yuko! :( my heart my bleeding heart! so sad :( she has nobody left! her only family is now gone  :cry: i got all teary
lucky takamina found her or else she might stay there all night crying D: and then she'll freeze to death during the night!

but LOL at that patient ! i literally laughed out loud during that part  :lol: and sasshi unintentionally scaring those poor nurses.... :smhid

Ohgadz. Is it just me fangirling when Maeda Ami came into the story? Or that she was mentioned during the first or second chapter? Or that she's sisters with Acchan? No? Yes? Oh well. :bigdeal:

I-expect-slight-TakaYuko-now. LOL. If there was more TakaYuko in the world, I don't know if it would overpower my love of Atsumina or not, but... ... AGHHGKEHRJHR THISFIC. UGH. :heart: lol.

Jurina. You're one perfect pimp. Even taking it as far as to kiss a nurse too (although she doesn't know it :P ). Damn. LOL. I love the idea of Jurina, Rena, and Sakiko being siblings, especially since they have the same last name.

Come to think of it, their last name is the most 'common' of the 48 family, right? I could only count that they currently have 2 Takahashis, 2 Maedas, 2 Oshimas, 2 Watanabes, 2 Oyas, and uhm... etc. I can't keep up with all the members. :dizzy:

YAY-RENA-AND-KUMI. AND-NAKANISHI-YUKA. I was happy that Shimada Haruka was brought into the story too. But I thought I was going to die if I read that it was Shimazaki Haruka instead. ...I probably would. :depressed:

LolSasshi. I wanted to know more about those "rare cases" from her. XD

I'm alive! Well sort of, been so busy with work and schooling all my brain can come up with is work related stuff soooooo I guess it makes updating this fic possible =P. Sorry for the long wait, I don't know if I'm starting to get a bit too technical and all with this story but ah well, its a good place for me to empty my mind  :twisted: 

I'll try to update the other stuff as well, been working on it but I want to get a bit more written before I post them... and.... I will be disappearing for a month of vacay. Not sure if I will get any writing done or if i will have internet so we will see.

I just have to put Aamin in a character that will appear more then just cameo in the story cuz she is awesome Mayuge-bon~. LOL don't be so fast to decide on the pairings because life is unpreditable right?  :twisted:  Still waiting for the last Matsui to make her entrance but for sure Jurina will be around to cause trouble for the doctors and nurses hahahahaha. Can't help it she is only a kid =). I don't know about the others but I really refer Shimada over Shimazaki. Once I can think up more Sasshi's "rare case story telling moments I'll add them"

         ~ Practice doesn’t make perfect, it brings permanence. Human’s are not made to be perfect ~



"Ahhh, so bored!" Sae complained lying on the couch in the respiratory department, holding the same magazine she picked up a few hours ago.

"Don't complain, it's nice to get some break time once in awhile." Miichan said from the desk while snacking on her sandwich.

"But we haven't had any calls since this morning." The energetic girl whined, missing the action and excitement their normally busy shifts bring.

"Say, I heard you know that new good looking young resident in emergency, heard you two went out for dinner together." Sasshi asked, looking up quickly from the computer to join in the conversation before turning back to the concert playing on youtube.

"WHAT! Where did you hear that from?"

"You’re overreacting, it's obviously true." Myao commented after just entering the room.

"You didn't even hear the question...."

*In Emergency*


"Mayuyu, you okay? You've been sneezing quite a bit today. Hope you aren't coming down with something." Dr. Aamin asked while still going over her patient's chart.

"I'm fine, I think someone is talking about me." Dr. Watanabe frowned.

"If you say so, but just keep clear of Dr. Matsui until her melon pan arrives, she's been in a bad mood since my sister and midgi-Mina decided they can't come in and need a cover." the young Maeda explained.

"Ah is that why she has this black aura surrounding her all morning?" Mayu asked, remembering how most of the staff has been trying to minimize contact with the dangerous doctor.

"I already called her sister and told her to bring in some melon pan. Those usually work and I really hope they would."

"Aamin! Are those charts done?!" The deadly voice transmitted throughout the department, bringing shivers down the spine for those who are waiting to speak with the owner.

"Right here, Dr. Matsui." With the speed of light, Aamin appeared beside the easily irritated doctor, showing her the charts for a sudden chart review of Aamin’s performance.

"Thanks." Rena said, realizing how snappy she is being while rubbing her temples to relieve the phantom ache she's been feeling all day.



*Some hour later*



"Good afternoon Hata-san.”

“Ah Jurina, here to visit your sister again?” The unit clerk, Sawako, asked pausing from deciphering the scribbles on the orders sheets.

“Aamin asked me to deliver some melon pan. Have you seen my sister?” Jurina asked. Since her last visit as a patient, Jurina developed a liking to the hospital and interest in this field of work. She would come often to visit her sisters and get to know the staff when Rena is busy.

“Hmmm I’m not sure. Akicha, have you seen Dr. Matsui?” Sawako asked the approaching nurse.

“I think she’s in the office." She replied, finally looking up from the chart she was reading. Out of reflex or safety response Akicha started to inch away from the young visitor. A flash of that incident once again appeared in her mind.

Sawako saw Aki's reaction and couldn't help but want to laugh, but being a professional unit clerk, she held it in and pretended to pick up something from under the desk. A few shaking and trembling moments later, Sawako straightened herself in the chair. Sawako found Jurina still standing there by the counter, spaced out looking across the room.

"Jurina? Dr. Matsui is in the office." She spoke up, trying to snap the girl back to reality.

"Hata-san, who's that girl?" Jurina suddenly asked.

"Which girl?" Curious as to who the girl is referring to, Sawako spun around in her chair in the direction Jurina is looking at.

"Oh that's...." She didn't bother to finish since the younger girl had already disappeared in a beeline towards the two young resident.

"Mayuge-bon!" Jurina shouted excitedly, hugging the taller girl. "Ah why are you so tall?" She pouted, having to look up to ask her question.

"Thank god, Juju we have been waiting for you to come save us!" Dr. Aamin returned the hug happily.

"Aamin, Aamin ,who was that with you just awhile ago?" Jurina asked looking side to side and behind the tall girl she is still hugging onto.

"With me? You mean Mayu? She is the new resident Watanabe Mayu." A confused Aamin asked, trying to stable herself from all the sudden movements caused by the koala still attached to her.

"Hmmm Mayu, where did she go?"

"To the lounge to get some water, why? .........Juju? Juri? Jurina!"  Aamin almost lost balance again when the koala suddenly ran off heading straight towards the lounge. In her mind, she thought of the best way to get even for being mostly ignored so Dr. Aamin made her way to the doctor's office to notify their lovely Dr. Matsui that there is melon pan calling for her in the lounge.



*In the lounge*



Dr. Watanabe stood there leaning on the back of the couch satisfying her thirst with a cup of water. The day has been busy and much more intense as Dr. Matsui made sure she stay on her toes, pointing out her flaws and things that could be improved. Of course she is grateful for all the constructive feedback but when they all come together like a landslide it is a little hard to adjust.

*Bang*

Her thoughts were interrupted by the sound of door opening and in poked a head looking around.

"Hello are you looking for someone?" Mayu asked not sure if she met or seen the girl before, she appears to be younger but Mayu can't be certain.

"Hi, I'm Matsui Jurina." Seeing the extended hand Mayu reacted like anyone would as a way for greeting.

"Watanabe Mayu, a new resident. You must be Dr. Matsui's ..." The young doctor wasn’t able to complete her sentence when she felt a tug on her arm causing her to lose balance.

“Chu ~ kekeke”

"MATUSI JURINA!" A roaring voice came from the doorway.

"Oops." Jurina quickly let go of the hand she is still holding, before had a chance to turn around, she felt intense pain tug on her ear and was then dragged out of the lounge.

"Ahhhh, Rena-chan ow, ow, ahhh."

"You are coming with me to Saki-nee." Rena said still dragging her younger sister by the ear, of course she made sure her melon pan is safely secured in her own hands first.

"Nooooooo, Rena not nee-chan, Aamin! Aamin! Save me!" The struggling Jurina begged as they passed by the emergency room where everyone watched the commotion. 

"Hmmm Mayu you okay? Did something happen?" Dr. Aamin asked when she found the girl standing still as a log in the lounge with one hand over her lips.

“......”

“Dr. Aamin and Dr. Watanabe stat to resuscitation bay! Dr. Aamin and Dr. Watanabe stat to resuscitation bay!” Hata Sawako’s voice came through clearly on the overhead kicking the two doctors back to reality.

“Abe, what’s the case?” Dr. Aamin asked as she watched the paramedics roll their patient in on a stretcher.

“Suicidal attempt, cutting and lost a lot of blood. The cheating girlfriend found her on the floor holding a knife and already bleeding out. The girlfriend reported the patient was still conscious when she found her but went unresponsive by the time we arrived.” The first paramedic, Abe Maria, reported while her partner help the nurse transfer the patient over to the hospital bed.

“Do we have a name? Any other medical history?” Dr Watanabe asked while she moved to assess the patient.

Maria took a look at their charting and answered, “Kashiwagi Yuki, female, previously healthy.”

“Thank you." the doctors replied before giving out some orders.

“Dr Aamin, we were able to insert two large IV’s on route, we started running some normal saline as well." The second paramedic replied after making sure the patient is secured with the nurses.

“Akicha, run another two liter of normal saline bolus from the IV’s wide open, insert another IV site if possible.” Dr. Aamin instructed.

“Yui can we get two units of packed red blood cells, and fresh frozen plasma, cross matched. She lost a lot of blood and will most likely need transfusion if hemoglobin comes back below 80." Dr. Watanabe added as Aamin moved to the bedside to do the initial assessment.

“Blood pressure is borderline, if the mean pressure drops below 50 then start some inotropes, if not let’s wait and see if we can fluid resuscitate her." Both doctors looked as each other and with quick doctor to doctor eye communication they nod in agreement to the plan they have set. With the immediate care plan set the emergency nurses moved around each other quickly getting everything set up and running.

“Hata-san, can you call Kuumin to resuscitation bay and do a set of blood on this patient, full lab work please." Dr. Aamin said to the unit clerk.

“Can we get a urine toxicology and screen as well?” Dr. Watanabe asked her senior as the idea came to mind.

“That is a good idea Mayu, Hata-san can you enter the order for urine toxic as well, there could be a possibility the girl took some drugs as well."

“Just make sure to monitor her fluid balance incase we overload her with fluid causing it to flood into the lungs and lead to respiratory failure." A happy Dr. Matsui came from behind the two young doctor’s causing them to jump from the sudden appearance.

“Good work both of you." Dr. Matsui finally gave them a compliment before leaving with her precious melon pans and a satisfied grin from being able to scare the little ones.

*BEEEP* *BEEEP*

The distinct alarm blared overhead causing all the lazing RT's to suddenly jump to their feets.

"CODE OB, LABOUR AND DELIVERY! CODE OB LABOUR AND DELIVERY!!"


"SAE! This is all your fault!" Minegishi, Sashihara and Miyazaki shouted as they quickly dropped what they were doing and ran out the door with Miyazawa Sae following behind.

"What's code OB?" She asked easily catching up to her running seniors.

"Obstetrics, pregnant mother in labour with high risk, all RT runs to them and decide how many need to stay after we arrive."

"Oh."

As they near the delivery room they can already hear the loud screaming and shouting that is typical of a mom in labour expect this time there is still 2 doors between them and the room. The volume of the screams of pain from the contractions escalated as each door opens for them to pass through. Sometimes they wonder why people would want to go through all this pain. It sounds 10 times worse than those being tortured or those horror movies.

"Haruu, Moeno? Did you bring her in?" Minegishi Minami shouted over to the two paramedics standing at the end of the hall right outside the room they are heading towards. Sasshi and Myao stopped outside the room to prevent crowding inside but staying within earshot so if needed they can be summoned. The other two ran into the room and straight to the warmer to make sure they have everything they need. Once they made sure the warmer is on, their respiratory equipment and suctioning devices are all set, they took a look around and ask the most important question.

"What happen? Baby full term?" The simple terms were shouted out towards the nursing staff making sure it can be heard over the screaming and thrashing mom.

"She just arrived by ambulance, full term, we found the baby with serious heart rate decelerations, worsening when she tries to push with contractions. Suspect the umbilical cord is wrapped around the neck, that's why... Haachan is holding the baby in place." One of the pediatric nurse replied.

After the nurse reported their findings both RT looked over to the mom in distress and sure enough at the end of the stretcher is a brave nurse, Katayama Haruka, standing there with her hand up....... holding the baby in place preventing it from being strangled to death. Sae turned back over to Miichan with eyes wide open trying to confirm what she just saw.

"Errm, what is your name?" Miichan asked the nurse that gave them the brief in attempt to change the focus as they wait for further instructions.

"Kasai Tomomi, one of the neonatal and pediatric nurses. Dr. Itano is on her way and will be here in a couple min, we are going to transfer into the operation room (OR) shortly when the room is ready for emergency caesarian section. chiyuu~"

"Sasshi! We are moving to OR, check the equipments there. We will stay with the mom." Miichan shouted over everyone towards the door then waited until Sasshi poked her head in and gave the okay sign. Of course there was also a second of surprise as she took in the scene.

"You did that on purpose." Sae said as a fact after seeing a smile grew on Miichan's face.

It didn't take long for them to move over to OR with the nurse hanging onto the stretcher, hand still holding the baby in place to prevent the cord from strangling this precious life. Miichan moved ahead to get in position beside the warmer waiting for the baby to be born.



*Dr Itano Tomomi’s POV*


"I'm Dr. Itano Tomomi and I will be in charge." I quickly introduced myself short and simple, before giving some instructions.

“Miichan you will be in charge of compressions and equipment, Tomomi I want you to tap me the heart rate and if need insert an umbilical line. Haachan you will be in charge of medication if needed."

I had to pause to read the name off the name tag before I could continue, “Sae you will be in charge of airway and breathing." All of this is to prevent any confusion later on when the baby arrives.

I check the clock waiting as I watch the time tick away slowly with the rest of the team. I can feel the tension begin to build up from the anticipation as each second that pass there is an increase chance that this baby won’t make it. The sound from the doppler clearly resonate each beat of heart rate it measures. Seventy, only seventy beats. I took a deep breath as this is the last stretch before the baby comes out. I can see the surgeon reaching in so any time now.

"Baby is out start the timer" That is my signal and as if someone press the start button, I took the newborn over to the warmer and within seconds I assessed the most important factors.

"No breathing, no muscle tone." I clearly called out what I see to the team as they dried her with some towels. I watched Tomomi flicks the sole a couple times hoping to stimulate some breathing efforts but no luck this time.

"Sae start breathing ventilation." I looked over to Tomomi who’s been working as me for a long time, without words she understand what I am asking her and started to tapping the heart rate for me to see.

"70 beats, heart rate is not rising, Miichan breathing tubes." I quickly got into position and took the tubes from Miichan then skillfully place the tube through the vocal cords and into the lungs. Thank god I practice my skills day and night back in the days, I always believed that practice only makes permanence and not perfect. No one is ever perfect you can only practice to make sure you don’t screw up as much. That’s why I believe in practice only makes permanence.

"Chiyuu~ Heart rate is below 60 now." Tomomi spoke up pausing her heart rate tapping alerting me that we should move onto the next step.

"Miichan two finger compressions, Tomomi I want that umbilical venous line, Haachan can you draw up some epinephrine for UV line. Remember 0.1-0.3mg only." It's already been 2 minutess since the baby is born and as each min pass there will be an increase chance of death or serious neurological problems, we are running low on time. If we lose the heart rate we will lose this baby within 10 minutes of that.

"Okay focus, coordinate the breathing and compressions, just like that. Good." From time to time I make sure to reconfirm what we are doing.

“Dr. Itano, this is 0.3mg of epinephrine." Katayama Haruka said as she passed me a labelled syringe.

“Doctor the UV line is ready for use."

Once Tomomi confirmed the UV line is in, I administered the medication quickly then watched the full resuscitation continue for at least another 45 seconds before prompting for another heart rate check.

“Tomomi what is the rate at now?” I asked in my mind I pray for this kid.
“75 beats doctor." She replied with a hint of bubbling joy.

“Good we are heading in the right direction, Miichan you can stop compressions. Sae keep breathing ventilation until the heart rate is higher than 100 beats per a minute (bpm).”

Within the next minute we watched the girl slowly make breathing efforts, weak but an effort is an effort. Her heart rate climbs steadily till it is over 100 bpm, but unfortunately her breathing isn’t able to maintain on her own without help. Otherwise her color, oxygen levels, heart rate and tone has now reach acceptable and sustainable levels.

After a couple more minutes when I am satisfied with the baby’s current state I decided it is time to take her to neonatal ICU (NICU) so we can closely monitor her in case anything else she needs more support.

“Heart rate is 124 bpm, breathing is borderline but showing improvements, oxygen levels look good, she is stable enough now, let’s take her to NICU so we can continue to monitor her, she might need some more breathing assistance so can one RT accompany us for the transfer.” I announced to the team.

Working with babies have always been very intense and satisfying when our efforts paid to allow innocent little life a chance to see the world. To me they have done nothing wrong, so to be finally able to take their first breath, see the first image and shout out to the world of their arrival is a blessing. The sound of their crying in a way is also the most beautiful sound one can wish for in a delivery room, the strength behind each cry is like a statement to the world they are ready to take you on. Sometimes I wonder why a baby covered in blood, in slime, in white sticky secretions can be the most beautiful thing in the world, but when I look at them there is just no words to describe how cute they are just like an angel.

“Congratulations you gave birth to a baby girl.”

So I actually finish writing this when I was away on vacay during my rare free days but because I had a derpy wifi I didn't get a chance to post this.

             ~ Don’t take health for granted, we care but we are not god. ~



“You’re finally awake."

I tried to open my eyes but the sudden rush of bright lights caused me to close them again. With my hand held above my face I slowly open my eyes again, allowing time for them to adjust. I look to the side for the person whose voice I heard as I started to become aware of my surroundings. Personally, I think it is a very cute voice to wake up to but that is the least of my concerns right now. At first I really thought I was dead but the light sound of beeping and familiar alarms told me otherwise.

“Emm where am I?” I asked, a question that I’m pretty sure I know the answer to but I need to confirm while my mind is still settling down from all the spinning.

“You are in Akihabara Hospital. Do you remember what happened before you lost consciousness?” The nurse standing by my bedside asked.

“I...” I remember very clearly why, it’s still fresh in mind. But I can’t bring myself to say what stupid actions I was about to resort to doing for such a stupid reason, for someone not worth the result.

I looked around to take in the surroundings, typical hospital emergency settings with drapes in between patient beds for a little privacy. The nurse beside me, she held a natural smile, not forced, not fake, it is that of someone who cares. Finally I look down towards the bandages wrapped neatly around my wrist; those wounds under the dressings will serve as a reminder of my stupidity for the rest of my life. A reminder of how I neglected the importance of life and how precious it is that each day should be a gift, not something that should be used to gamble. I have no right to give up. Compared to some of the people out there, I am very lucky to have what I have, just only a minor setback that’s all.

“Are you okay? Miss Kashiwagi are you feeling any pain?” The nurse asked again, I can hear the concern she has from just the way the question came out, the way her expression changed.

“I’m okay, Thank you, Thank you for saving me, Takajo-san.” I am truly grateful and I owe them my life for saving me.

“You’re welcome, we only did what is right together as a team.”

“Thank you.” I repeat once again.

“Kashiwagi-san, if there is anything you need please call me, I am your nurse for today.” She said. “You are still weak from the blood loss so please remain in bed.”

“TakaMini! Don’t worry, I will be here to watch over Yuko.” I said trying to get my stubborn best friend/housemate out of the house. “You have to go to work or Rena will rage. Aamin said even melon pan isn’t working anymore.”

I already knew this would be a difficult task so I took the time to prepare everything, pack her bags and lunch before working onto moving the body. Initially there was quite a bit of resistance and struggle but eventually she gave in.

“I guess you are right.” Takamina said after glancing back at the closed guest door. *Sigh* Take care of her while I am gone.”

“You don’t need to tell me that, I’m a doctor remember, the best of the best.” I joked, pretending to boast about myself while patting her head.

“Hey! I’m not a kid." she playfully swatted my hand away. “plus we all know I’m the best.” There we go, much better.

After what seemed like forever, I, Maeda Atsuko, have finally managed to complete the hardest task of the day: ‘Get Dr. Takahashi to work’ as my sister named. Since the night we found Yuko outside in the parking lot, Takamina has been taking time off to spend time consulting her friend. Normally I would fully support Minami but when she begins to neglect her responsibility as a doctor I can’t just stand by and watch. I understand that Yuko needs someone to be there for her, help her cope and transition through a challenging time in life. Personally I really want to help her as well so I came up with the plan to stagger our rotation so at least one of us can stay with Yuko.

“Yuko? It’s Acchan, I made you some rice porridge.” I said while trying to balance the tray of food as I knocked lightly at the door. It’s a new skill I learned after almost burning myself with soup last time.

I waited but when I got no answer, I gently opened the door and found Yuko sitting there on the bed, knees hugged to her chest while she stared out the window. I placed the tray down on the nearby table and walked over to the lost soul. I know we haven’t known each other for a long time, only meeting a few times through Minami during school days but from my memory each and every time I see her, she held a smile, a confident smile. There is also that playful smile when she is causing trouble in class, the perverted smile when she runs around in the locker rooms and the gentle smile she showed me when I was insecure with my abilities to live up to my families expectations. She was a very bright student if only circumstances didn’t sway her from her path but even so she never backed down from a fight, she never showed fear and never showed any weakness... until now.

“Yuko, I brought you something to eat, just a little, even if you just eat a little.” I said softly, hoping to get her attention. “Yuko, please?” I asked reaching out for her.

“Why did she have to go? Was it because of me? Was I bad so she left me too?” The way her words came out broke my heart. I pulled her in for a hug making sure she is secured tightly in my arms so she can feel my warmth and know she isn’t alone.

“Don’t cry Yuko, your grandmother doesn’t want to see you like this, she has always been very proud to have you as her granddaughter. It’s just her time is up so she has to go. She will never leave you alone, up above she will always be watching over you.”

“Really? Can she see me now? I miss her.”

“Of course, remember Yuko, your grandma will always live in your memories, never forget them and treasure the moments you’ve had together. Make her proud and live the rest of your life to the fullest. Show her the little girl she raised has finally grown up.”

After that Yuko started to eat, little by little she finished the bowl of rice porridge.

“Thanks Acchan.”

“You know that is the first time you called me Acchan.” I said with a smile, causing her to finally look me in the eye.

“Your words, you reminded me what I forgot. For my grandma I need to live on, she is watching me.”

I gave her another heart warming hug, happy that she is finally opening up to us. Just wait till I call Takamini and tell her the good news.

“Well it’s not like I can call you Takaman, you threw her out the door earlier."

I snapped out of my internal celebration when I heard her whisper that softly and I had to check twice but for sure it’s that smile, her silly smile!! Better yet she makes fun of mini too! Wait...

“You heard all that?” I asked remembering the embarrassing argument I had this morning. Those little minor details that we shouted at each other would’ve been best kept in the dark but unfortunately a third soul now shares the knowledge. 

“You two are loud you know, but really thank you, you and Takamina."

“Thank the mini yourself."


“Mini? Hahahahaha” For the first time we both laugh our hearts out, knowing Takamina will always be there to bring us laughter even when she isn’t physically around.

Shimada Haruka and Abe Maria stood by their ambulance outside of the hospital enjoying a couple of hot coffee as they watched the sky slowly darken after a busy day. After a day coming in and out of the hospital and driving around the city to serve those that needed the care and those that abuse the service, they finally get to settle down for a break from the busy life of a paramedic. In their minds they wonder if tonight will be a night where they can be off on time and not run overtime like the typical overworked paramedics.

People might not understand them, and don’t realize how valuable they are. Being the first to take a call to an emergency with minimal information then to be the first to arrive on scene and take immediate action to stabilize and act to preserve life. The stress and responsibility that comes with each time they are dispatched and the hours and time they have to work. When life is on the line there is no “Can it wait?” There is no “I’m on break.” because nothing is more valuable than life.

Teamwork is an important aspect of their life as they work in pairs with a partner, the time they spend together the trust that is required shows in the care they provide. So at this moment in time to Haruka and Maria, to be able to stand there and enjoy a cup of coffee is like a treat. It is also this habit of taking breaks outside the paramedic entrance by the main emergency entrance that occasionally saves a life that find their way to the hospital.


“HELP!”

"SOMEBODY HELP US!"

Shimada and Abe paused to look over to the car that suddenly pulled over outside the emergency entrance, out came two young men dragging a third out from the back of the car. The girl who started shouting for help came out of the driver seat screaming loudly alerting some of the staff from inside triage. Both paramedics quickly jumped into action as Haruu ran towards the group instructing them to lay the unresponsive young man on the ground so she can check for breathing and pulse. Just from appearance this young man looks pale and somewhat cyanotic with no muscle tone, part of the arm already start to appear mottled as well. Maria on the other hand ran to grab their stretcher to help transport the patient inside, the second she saw them drag another person out of the car she knew this is a serious case. When she turned around she found Haruu have already started CPR, so quickly and smoothly they shift the patient onto the stretcher and wheeled him in with Haruu riding on the stretcher to maintain compressions hoping to maintain blood circulations to the vital organs. Maria on the other hand started to manually ventilate the patient with a bagger and mask while a second paramedic team came over to help move them inside.

"RESUS BAY IS FREE!" They heard head nurse Kuramochi Asuka shouted as they rolled through the doors.

"Yui get the board under him, Akicha attract the monitors, Mizuki get ready to take over for Haruu. Have we called RT?" Dr. Maeda Aamin shouted over the busy bodies all getting in position in hope to save this young soul.

"Respiratory STAT to Resuscitation Bay! Respiratory STAT to Resuscitation Bay! "

Unfortunately for Aamin, Dr. Matsui is busy doing some procedure with Dr. Watanabe on the other side of the unit and Dr. Takahashi called and said she is running a bit late so she is left on her own to handle this case in the meantime.

"Sasshi here!" A voice shouted across the room as the body came running through the door.

"Sasshi, get stuff ready we will be intubating now." Dr. Aamin made her way to the head of the bed and waited for the lone RT to be ready with the breathing tube.

"How long since we took over and started compression? Yui can you start an IV and run it wide with normal saline, Akicha prepare epinephrine, Mizuki stop compression when I intubate and check pulse." Dr. Aamin spoke out loud and clear for everyone present to hear.

"I am ready to restart compression." Haruu said for everyone to hear.

"Dr. Aamin I'm ready, here is the laryngoscope." Sashihara Rino said while handing the lighted blade to the doctor, "I have the suction and breathing tube, let me know which one you want."

Aamin inserted the lighted blade into the patient's mouth, "Hold compressions and check pulse." She said without looking up, her eyes concentrated in looking for the vocal cords, upon spotting her target she lifted her free hand, "Sasshi, breathing tube. Mizuki, do we have a pulse?"

The process of inserting the breathing tube took only a couple second and its placement was confirmed quickly after listening over the lungs and abdomen area. "Size 8 tube at 24 cm, placement confirmed." Sasshi said for Mocchi to chart down the information. Satisfied with its placement Aamin moved out from the head of the bed to let their respiratory therapist take over the breathing aspect so she can concentrate on the big picture.

"No pulse, doctor!" Kuwabara Mizuki shouted over and above all the other sounds in the busy room. Shimada once again showed off her strong arms by quickly resume effective compressions while they waited for more orders.

"Aki one amp of epinephrine! IV if possible, do we have a line?" Aamin asked following the acute cardiac life support algorithm.

"IV is in and ready for use doctor." Yokoyama Yui shouted from her corner of the room.

"One amp EPI given." Aki shouted to Mocchi who is busy writing everything down to keep track of drugs and treatments given, especially to note when 3-5 min is up so they can repeat the doses of medication.

Time continue to count down as they repeated the cycle in hopes to restart the heart despite the unknown down time. Each minute that passed, their chances of success drops significantly. Within the 20 min they have had palpable pulses for a short period of time after they gave more drugs and fluid, but the pulse will slowly fade and Aamin is starting to lose hope and is unsure how to continue. If only
Takamina or Rena is available for some experienced advice and judgement, all Aamin know is that it is too early to give up for this young patient.

"Have you checked for cardiac tamponade? You can try to infuse epinephrine and vasopressin, it should hold the pulse and buy you some time." A voice spoke up from the side.

The sudden intrude of an unfamiliar voice caused everyone to look up, finding Kashiwagi Yuki standing slightly off to the side of the resuscitation bay in a patient gown holding onto her IV pole. She was peeking in from the side of where the curtains are drawn closer to her own hospital bed.

"I'm sorry Miss you can't be here." Mocchi spoke up as this is against the rule of confidentiality.

"Mocchi, Kashiwagi-san is an internal medicine doctor!" Takajo Aki suddenly spoke up remembering the history she read on her patient, Kashiwagi's chart.

"Wait! Mocchi let her in." Dr. Aamin said after thinking over what the older girl had suggested and what Aki reported, "She is right, start the infusion and can I get the ultrasound machine?"

"Kashiwagi-san are you familiar with using ultrasound for tamponade? I haven't performed that diagnostic check alone before." Aamin said in honest, as there is no room for mistakes and not the right time to make guesses.

"May I have some gauze?" Kashiwagi Yuki asked as she moved her hand over to pull out the single IV in her forearm so she can ditch the bulky IV pole and slip past everyone to the bedside.

"We now have a pulse." Mizuki said while she keeps her hand placed over the carotid artery so she will know if they lose the pulse again. Yui and Aki work at super speed to get the infusion started seeing how it takes a bit of time to prepare.


With skills and accuracy, Yuki demonstrated to Aamin how and where to place the ultrasound probe to look for any bleeding and tamponade around the heart, at the same time she checked the heart for activity to see it continue to pulse weakly.

"No tamponade." Yuki said as she passed the probe back to one of the nurses.

"What else can we do, doctor?" Dr. Aamin asked since she still lacked experiences for these situations.

"Let's go by the algorithm, looking at the reversible causes. We have ruled out tamponade." Yuki started her explanation of what they should do next.

"We have been giving fluid and started infusion so that should reverse hypovolemia if that was the problem." Aamin reasoned out as Yuki nodded for her to continue, "patient is intubated and oxygenated so the problem is no longer hypoxia, although that could've been a problem and may lead to anoxic brain injury."


"What about acidosis and hyper or hypokalemia?" Yuki brought up the next question for the younger doctor.

"I've given 2 amps of bicarb during the resuscitation cycle, but we haven't had any blood work or toxin screen done as he wasn't stable enough." Aamin told Yuki.

"Is patient more stable now? What else is on the list for reversible causes?" Yuki asked another question for the younger doctor to work out as she notices the patient appears to be stable at the moment from the drugs.

"We have a pulse now, airway is secured, blood pressure is temporarily stabilized by the infusions, and so we should get the screen and complete blood and metabolic works done then check for pulmonary and cardiac thrombosis." Dr. Aamin said while she waited for the experienced doctor to confirm her words.

"Komori call Kumi then CT, we need a stat CT head and chest scan." Dr. Aamin told today's unit clerk, Komori Mika.

"Good job Aamin. Thank you Kashiwagi-san." Dr. Takahashi Minami said from behind the two, causing them to jump from surprise.

"Takamina!!!" Aamin shouted in surprise then she remembered where they are, "Dr. Takahashi, about time you arrived, why are you so late?"

"I got here a while ago but you and Kashiwagi-san seemed to be handling the situation quite well so I stayed back to watch." Dr. Takahashi explained as she looked over the patient monitors and charts for what has been done.

"Do we have a history of what happened?" Dr. Takahashi asked Mocchi while she reviewed the resuscitation charting.

"Dr. Takahashi, we don't know the full story of what happened yet, the kid's friends drove him in and stopped in front of the entrance, Maria and I just happen to be taking our break so we started CPR immediately after seeing the state he is in." Shimada Haruka reported over to the doctor and nurses now that they finally have a bit of time.

"Doctor, according to the patient's address it would take them at least 10-15 min to drive here from home." Komori told the team as she pulls up the patient information to facilitate a proper admission with patient file to log all the charting and results of this visit.

Yuki excused herself back to her bed closest to the resuscitation bay seeing how the patient is now stable and the senior doctor has returned, so her expertise is not required anymore. Originally she debated if she should intervene from her bed while listening to the whole resuscitation but in the end she decided that at most they would just kick her back to her bed if they don't need the help. Now she is glad she decided to take that step and offer some of her experience and knowledge, being able to help made her feel that much more important to continue living on. She continued to watch nurses go in and out through the curtains to care for the patient while the two doctors go over what has been done and what else is needed to be done. In her mind Yuki thought poor kid, if only the friends know to call for an ambulance instead of driving him in. At least then they would be instructed to start CPR or wait till paramedics arrive to start CPR but by driving the kid in there is at least another 10-15 minute without blood circulation. Each min of early start of CPR would give the patient a better chance of survival but seeing how the treatment has been progressing there is a high chance of anoxic brain injury and he might not even live through the night.

I'm not an expert in medicine.. But the explanation and description of every information in there is very astounding~!!!! (I like science  XD XD XD )

I just thought, are you a graduate in medicine, that you know all of these?? It requires 10 years of study, not including in many specialty....

I will put this in my favorite fic list.... The other one who has the same genre is Amnesia (Cardiac Emergency)....  :lol: :lol: :lol:
