It's pretty straightforward! JPEG from your computer, then your e-mail. Solo faces, IIRC. Link your photo once it's up! They give you a catalog number to search for it on the main mosaic page.

I swear I saw some images getting repeated across the mosaic, but perhaps they went over and changed it now~
They've emailed me twice now with details on where to order and stuff, I suppose that might be them telling me, then reminding before they stop making them or something- but I can't order one OAO;
